STUDENT ROTATION TRAINING AGREEMENT 
 
 
This Student Rotation Training Agreement (“Agreement”) sets forth the terms of 
the clinical educational rotation of students from the Arizona Board of Regents acting on 
behalf of The University of Arizona Health Sciences1 pursuant to Arizona Revised 
Statutes Title 15, Chapter 13, Article 2, Section 1625.B.2 (“AHSC”), whose principal office 
is located at 1670 E. Drachman St., 9th Floor, Tucson, AZ 85721, to Maricopa County, by 
and for its Department of Public Health (“MCDPH”), whose principal office is located at 
4041 N. Central Ave., Ste. 1400, Phoenix, AZ 85012 for educational training and 
experience.  
 
1. Rotation Goals and Objectives 
 
A. The educational goals and objectives for any rotation will be provided by AHSC to 
the applicable MCDPH preceptor prior to the start of the rotation. If these goals 
and objectives are acceptable to MCDPH, the MCDPH preceptor will sign a 
statement agreeing to meet the goals and objectives of the rotation and will return 
a copy to AHSC.  
 
B. AHSC shall retain full control and responsibility for the overall education of the 
students.  
 
C. The purpose of this Agreement is to establish coordinated clinical educational 
programs and guidelines for the education and training of health sciences 
students, externs, interns, residents and/or fellows from AHSC, (“Student or 
Students”).  
 
D. The accreditation bodies (“Accreditation Agencies”) responsible for the 
accreditation of the academic programs within each of these colleges, include but 
are not limited to the following:  
 
1.D.1. Accreditation Review Commission on Education for the Physician 
Assistant, Inc. (“ARC-PA”) for the Physician Assistant Program.  
 
1.D.2. Commission on Accreditation in Physical Therapy Education (“CAPTE”) 
for the Doctor of Physical Therapy Program.  
 
1.D.3. Accreditation Council for Genetic Counseling (“ACGC”) for the Genetic 
Counseling Program.  
 
1.D.4. Accreditation Commission for Midwifery Education (“ACME”) for the 
Midwifery Program.  
 
1 Including but not limited to: College of Health Sciences; College of Medicine-Phoenix; College of 
Medicine-Tucson; College of Nursing; College of Pharmacy; and College of Public Health.

1.D.5. Liaison Committee on Medical Education ("LCME") for the medical 
student curriculum.  
 
1.D.6. Commission on Collegiate Nursing Education ("CCNE") for nursing 
programs.  
 
1.D.7. Council on Accreditation of Nurse Anesthesia Educational Programs 
(”COA”) for the nurse anesthesia programs.  
 
1.D.8. Accreditation Council for Pharmacy Education ("ACPE") for the pharmacy 
programs.  
 
1.D.9. Council on Education for Public Health ("CEPH") for the public health 
programs.  
 
2. Responsibilities of AHSC for Rotations 
 
A. Provide qualified faculty and a properly structured educational experience to 
Student Participants in accordance with the accreditation standards of the 
applicable Accreditation Agency identified in Section 1D.  
 
B. Agree to notify Students of MCDPH’s requirement that Students participating in 
the clinical educational program at MCDPH adhere to existing rules and 
regulations of MCDPH.  
 
C. Agree to notify Students of MCDPH’s requirement that each Student participating 
in the clinical educational program at MCDPH meets the same health standards 
that are required of all employees at MCDPH. MCDPH may request upon 
reasonable notice a physical examination and/or laboratory or other diagnostic 
tests for any Student. The Student shall have the option to either agree to such 
physical examinations, labs, and tests or AHSC will withdraw the Student from 
MCDPH. MCDPH and AHSC shall not be responsible for any cost or charges 
incurred for such physical examinations, labs, or tests.  
 
D. Agree to notify Students of MCDPH’s requirement of Students’ duty not to release 
any material relative to practical experiences that has not been approved by 
MCDPH for release for publication or to third parties, or that is confidential by law.  
 
E. Agree to notify Students of MCDPH’s requirement that that Student dress and 
grooming shall be appropriate for MCDPH and for health care workers.  Students 
will comply with the MCDPH dress code.  
 
F. Notwithstanding any provision to the contrary, AHSC agrees that the Students 
enrolled at MCDPH shall not be employees or agents of MCDPH or Maricopa 
County, and shall not be entitled to any defense or indemnity under Maricopa 
County’s Declaration of Trust for Maricopa County, Arizona, Self-Insured Risk 
Trust Fund, benefits, conditions, terms or rights of MCDPH employees including

workers’ compensation, medical or hospitalization insurance or benefits, pension, 
retirement or vacation pay, credit or other benefits or rights of MCDPH employees. 
MCDPH may, at its own election, decide to provide Students a stipend. MCDPH 
employees employed by AHSC as faculty members on their own time (non-
MCDPH pay status) will be subject to the same restrictions.  
 
G. Upon MCDPH’s request, AHSC will remove from MCDPH any Student for 
unsatisfactory performance or failure to follow MCDPH's administrative and 
patient care policies, procedures, rules and regulations, or any applicable laws of 
the State of Arizona.  
 
H. Reimburse MCDPH for damage to equipment or supplies, provided MCDPH 
provides evidence that such damage was caused by Students or faculty 
observers.  
 
I. 
Designate a person or persons to assist in the coordination of Student learning 
experiences, including planning with MCDPH staff to assign Students to specific 
learning experiences that are appropriate for the program(s).  
 
J. Provide at least 30-day’s advance notice for Students assigned to a rotation.  
 
3. Responsibilities of MCDPH for Rotations 
 
A. Provide training experiences and make available the related facilities at MCDPH 
for Students enrolled in any program. MCDPH, together with its professional staff, 
has complete and total authority and responsibility for control over the details and 
means for the training experiences.  
 
B. MCDPH staff will teach and supervise Students while assigned to MCDPH 
experiences. Any direct patient care or client services provided by Students shall 
be under the direct, immediate and exclusive supervision of qualified staff 
employed by MCDPH. All patient/client orders and staff instructions, methods, 
techniques and procedures initiated and/or performed by Students will be subject 
to prior review, approval and counter signature by appropriate MCDPH personnel. 
MCDPH, together with its professional staff, agrees to maintain exclusive control 
over MCDPH's services to its patients/clients and be the responsible party for all 
decisions related to such services. Students will not have independent authority.  
 
C. Designate a person or persons or MCDPH unit to assist in the coordination of the 
Students’ learning experiences, including planning with MCDPH staff for the 
assignment of Students to specific learning experiences which are appropriate for 
the Students’ program. MCDPH agrees to notify its staff of the obligations covered 
by and agreed to in this Agreement.  
 
D. Keep AHSC informed of changes in policy that affect Students.  
 
E. MCDPH will provide for the prompt and appropriate transfer of a Student to an 
appropriate facility in the event a Student requires emergency medical care and/or

treatment for a life-threatening situation. In such case, MCDPH does not assume 
the associated expenses for such medical treatment, including costs of transfer 
and further treatment.  
 
F. MCDPH agrees to provide Students any additional training beyond training in 
Human Blood and Body Fluid Precautions that meets OSHA Regulations; if 
required by MCDPH 's facilities and programs.  
 
G. MCDPH agrees to promptly provide the AHSC a written statement describing the 
reason or reasons for the removal of a Student due to the Student’s unsatisfactory 
performance or failure to follow Student’s administrative and patient care policies, 
procedures, rules and regulations, or any applicable law of the State of Arizona.  
 
H. In accordance with accreditation guidelines by the appropriate Accreditation 
Agency identified in Section 1D, MCDPH will comply with, at a minimum, the 
following requirements:  
 
3.H.1. Ensure responsibility for the control and operation of MCDPH’s facilities 
and its programs.  
 
3.H.2. Cooperate with AHSC and acknowledge the AHSC’s sole authority with 
respect to academic programs.  
 
3.H.3. Use its best efforts to ensure that its environment promotes the 
development of appropriate professional attributes in the Students 
assigned thereto.  
 
I. 
MCDPH will permit representatives of AHSC and academic accreditation 
agencies to inspect its facilities, the services made available for the placement of 
Students, Student records, and other information relevant to the placement.  
 
J. MCDPH will, at the commencement of a Student’s placement, provide the Student 
with an orientation about MCDPH’s policies, procedures, standards and practices 
relevant to the Student’s placement including information regarding safety and 
emergency procedures. If applicable, MCDPH will also provide to Students HIPAA 
(as defined herein) policies and procedures that are unique to MCDPH’s facilities.  
 
K. To the extent MCDPH generates or maintains educational records related to 
participating Student(s), MCDPH agrees to comply with the Family Educational 
Rights and Privacy Act ("FERPA"), to the same extent as such laws and 
regulations apply to AHSC and shall limit access to only those employees or 
agents with a need to know. For the purposes of this Agreement, pursuant to 
FERPA, AHSC hereby designates MCDPH as a school official with a legitimate 
educational interest in the educational records of the participating Student(s) to 
the extent that access to AHSC’s records is required by MCDPH to carry out the 
clinical educational program.

L.  MCDPH will provide for the prompt and appropriate transfer of a Student to an 
appropriate facility in the event a Student is exposed to a potentially infectious 
agent or hazardous material (i.e. an exposure by skin, eye, mucous membrane, 
or parenteral contact with blood or other potentially infectious materials that may 
result from the participation in the clinical training activities or an exposure by 
inhalation, skin, eye, or mucous membrane to substances defined as hazardous 
chemicals that may be present in the training site). MCDPH will not be responsible 
for any post-exposure testing and further prophylactic drug treatment. Students 
are responsible for costs of the initial assessment, initial prophylactic treatment, 
and necessary tests including labs related to the exposure. MCDPH will provide 
copies of all incident reports documenting the circumstances of the exposure to: 
University of Arizona Risk Management Services, P.O. Box 210300, Tucson, 
Arizona 85721-0300, Attn: Director of Occupational and Environmental Health 
and Safety.  
 
4. Mutual Responsibilities of MCDPH and AHSC 
 
A. The personnel of MCDPH and the faculty of AHSC assigned to the program will 
confer at such times as may be mutually agreed upon to evaluate the educational 
program.  
 
B. The personnel of MCDPH and the faculty of AHSC assigned to the program shall 
cooperate in providing a sound educational environment.  
 
C. Students are assigned for practical training experiences in the appropriate 
MCDPH unit. Both parties shall mutually agree upon the number of Students and 
the appropriate MCDPH unit for training.  
 
D. Upon request, each party will complete program evaluation forms for Students, 
faculty, or learning experiences.  
 
E. Students will not have implied, presumptive or apparent authority or independent 
health care responsibilities for MCDPH’s patients and clients and are not to be 
considered as ostensible agents, independent health care providers, patients’ 
advocates, MCDPH's employees or agents, or individuals practicing in an 
extended role while participating in an education program pursuant to the terms 
of this Agreement. 
 
F. Students will not share in either the allocation of the duty of care responsibilities, 
determination of service utilization, or establishment of criteria for standard of care 
for MCDPH 's patients. 
 
G. The withdrawal, dismissal or removal of a Student in accordance with Section 3G 
of this Agreement, will not affect continuation of this Agreement or the continued 
training of other Students.  
 
H. For purposes of the Health Insurance Portability and Accountability Act (HIPAA), 
AHSC and MCDPH acknowledge that Students are part of MCDPH’s “workforce,”

as defined in the HIPAA Privacy Regulations at 45 C.F.R. 160.103, and as such, 
no Business Associate agreement is required between AHSC and MCDPH. 
 
I. 
In keeping with all federal and state rules and regulations regarding patient/client 
confidentiality, AHSC will notify the Students that they are responsible for 
maintaining the confidentiality of patient/client information. AHSC expects that 
MCDPH will not permit Students to access, or have the right to review, any 
medical or epidemiological record, except where necessary in the regular course 
of the training program covered by this Agreement or in furtherance of any 
litigation arising out of Student’s affiliation. Additionally, because of AHSC’s 
potential reliance upon MCDPH for information required by federal, state and/or 
accreditation requirements, MCDPH agrees to provide AHSC access to such 
information for those purposes. AHSC will advise Students that they are not 
permitted to discuss, transmit or otherwise communicate, in any form, any 
patient/client information of a personal nature, medical or otherwise, obtained by 
the Students, except as a necessary part of the training programs covered by this 
Agreement. AHSC will affirm that all Students have received training required by 
Federal Rules and Regulations for the protection of patients' protected health 
information ("PHI") and that departmental records of completion of such training 
will be available to MCDPH upon written request. AHSC expects that MCDPH will 
provide any additional training to Students as members of its workforce. AHSC 
agrees to notify Students of such requirement and MCDPH will provide AHSC with 
records of such attendance.  
 
5. Term and Termination 
 
A. The term of this Agreement shall be from January 29, 2025, through June 30, 
2029. The Agreement may be renewed for additional terms by the mutual consent 
of both parties.  
 
B. This Agreement may be terminated by either party upon thirty (30) days prior 
written notice to the other party; provided, however, Students participating at 
MCDPH at the time of termination shall be given an opportunity to complete their 
participation.  
 
6. Insurance  
 
AHSC is insured under the provisions of Arizona Revised Statues §41-621, as 
administered by the Arizona Department of Administration, Risk Management 
Division. Maricopa County, which includes MCDPH, is self-insured pursuant to 
Arizona Revised Statutes § 11-981 as governed, defined, and controlled by the 
terms of the Revised Restated Declaration of Trust for Maricopa County, Arizona 
Self-Insured Risk Trust Fund, approved by the Maricopa County Board of 
Supervisors and maintained and administered by the Maricopa County Risk 
Management Department. Insurance coverage provided by these statutory 
programs is deemed sufficient for the purposes of this Agreement.  
 
7. Miscellaneous

A. AHSC and MCDPH shall not discriminate against any individual on the basis of 
race, color, sex, pregnancy, childbirth or medical conditions related to pregnancy 
or childbirth, political or religious affiliation or ideas, culture, creed, social origin or 
condition, genetic information, sexual orientation, gender identity or expression, 
national origin, ancestry, age, disability, military service or veteran status, or 
marital status, whether as to employees, applicants for employment, students, 
faculty, or clients. To the extent applicable to this Agreement, both parties agree 
to comply with all state and federal employment laws, rules, regulations, and 
executive orders, including Executive Order 2009-09 as amended by Executive 
Order 2023-01, relating to equal employment opportunity and non-discrimination. 
Additionally, in the course of carrying out duties pursuant to this Agreement, 
MCDPH agrees that, if a Student makes an allegation or complaint either to 
MCDPH or to AHSC that s/he has been subjected to unlawful discrimination by 
MCDPH, MCDPH agrees to cooperate with AHSC to investigate and resolve such 
allegations or complaints.  
 
B. Nothing in this Agreement shall be deemed or construed as creating a 
relationship of principal and agent or of partnership or of joint venture between 
the parties. Students shall not be considered servants, agents, or employees of 
MCDPH.  
 
C. This Agreement shall be construed and enforced in accordance with the laws of 
the State of Arizona. Exclusive venue and jurisdiction for any actions or 
proceedings related to or stemming from this Agreement will be in the state or 
federal courts sitting in Phoenix, Arizona.  
 
D. This Agreement shall be binding upon and shall inure to the benefit of both parties 
and their respective successors, heirs, assigns and legal representatives. This 
Agreement may not be assigned in part or whole without the prior written consent 
of both parties.  
 
E. The Parties have authorized the undersigned individuals to execute this 
Agreement on their behalf.  
 
F. 
This Agreement contains the entire understanding of the parties relating to the 
subject matter of this Agreement. Any prior agreements, promises, negotiations 
or representations between the parties, either oral or written, relating to the 
subject matter of this Agreement that are not expressly included are of no force 
or effect. Any modifications or amendments must be agreed to by both parties in 
writing.  
 
G. During the term of this Agreement, the parties shall take such actions, including 
revising this Agreement, as necessary or advisable to comply fully with all laws, 
rules, and regulations applicable to the performance of this Agreement, including 
without limitation the Health Insurance Portability and Accountability Act 
(“HIPAA”) and the FERPA.

H. Service of all notices under this Agreement will be sufficient if mailed to that 
party’s address set forth herein, by overnight courier service or by certified or 
registered mail, return prepaid, addressed to the party as follows:

Notices given in the manner described above will be deemed received by the 
addressee Party on the third business day after the date of mailing. 
 
I. 
Each party (as Indemnitor) agrees to indemnify, defend and hold harmless the 
other party (as Indemnitee) from and against any and all claims, losses, liability, 
costs, or expenses (including reasonable attorney's fees) (hereinafter collectively 
referred to as "claims") arising out of bodily injury of any person (including death) 
or property damage, but only to the extent that such claims, which result in 
vicarious, derivative liability to the Indemnitees, are caused by the act, omission, 
negligence, misconduct, or other fault of the Indemnitor, its officers, officials, 
agents, employees, volunteers, or (as to AHSC) Students.  
 
J. 
Compliance Requirements for A.R.S. § 41-4401, Government Procurement; E-
Verify Requirement:  
 
i. 
AHSC warrants compliance with all federal immigration laws and 
regulations relating to employees and warrants its compliance with A.R.S. 
§ 23-214(A), which states, “After December 31, 2007, every employer, 
after hiring an employee, shall verify the employment eligibility of the 
employee through the e-verify program and shall keep a record of the 
verification for the duration of the employee’s employment or at least three 
years, whichever is longer”.  
ii. 
A breach of warranty regarding compliance with immigration laws and 
regulations shall be deemed a material breach of this Agreement and 
AHSC may be subject to penalties up to and including termination of the 
contract.  
iii. 
Failure to comply with a state audit process to randomly verify the 
employment records of contractors and subcontractors shall be deemed a 
material breach of this Agreement and AHSC may be subject to penalties 
up to and including termination of the Agreement.  
iv. 
The State of Arizona retains the legal right to inspect the papers of any 
employee who works on this Agreement to ensure that the contractor or 
subcontractor is complying with the warranty under Paragraph i.

K. This Agreement may be cancelled in accordance with A.R.S. § 38-511.  
 
L. The parties recognize that the ability to perform under this Agreement may be 
dependent upon the availability of funds. The Parties agree that if an appropriation 
is reduced or canceled, either Party may reduce its scope of involvement in this 
Agreement or cancel the Agreement without further duty or obligation by 
delivering written notice to the other party as soon as reasonably possible after 
the unavailability of funds comes to a party’s attention.  
 
M. The parties acknowledge that disputes arising from this Agreement may be 
subject to arbitration in accordance with applicable law and court rules. 
 
N. Special Provisions:  
 
The Special Provisions, which may be attached, will set forth additional particular 
responsibilities of the parties hereto and, are, by this reference, made for all 
purposes a part of this Agreement. They are not intended to limit the parties from 
developing Special Provisions for other programs covered by this Agreement. In 
cases where the representations of the Special Provisions differ from those in the 
General Provisions, the representations in the General Provisions will control.  
 
Special Provisions will be added or amended as necessary to reflect changes 
including work statement activities, reporting periods, frequency of reports, report 
formats, payment, personnel or supervisor changes, and will be attached to and 
become a part of this Agreement when such Special Provisions have been 
mutually agreed to and signed by the appropriate and authorized persons from 
each party.  
 
O. The University of Arizona, College of Medicine-Phoenix’s Diversity Mission 
Statement (“Diversity Statement”) is attached and incorporated into this 
Agreement. The Diversity Statement is applicable only to AHSC. It does not 
impose any duty, obligation, or liability on Maricopa County, MCDPH, or any of 
their officers, officials, agents, employees, or volunteers.  
 
The persons executing this Agreement represent and certify that they have the full power 
and authority to enter into this Agreement on behalf of the entities for which they are 
signing.

Special Provisions 
 
I. 
These Special Provisions are intended to cover, but not be limited to, the training 
of Global MD students (“Program Participants”) from the College of Health 
Sciences, University of Arizona while they are at MCDPH.  
 
II. 
Items of Agreement  
A. Access to Professional Development  
 
1. Newly recruited clinical teaching faculty may be unfamiliar with 
competency-based assessment strategies used in the clinical clerkship 
phase of the University’s College of Health Sciences Global MD Program 
(“Global MD Program”) and will require adequate training. A 
development program will be provided by the Global MD Program for 
MCDPH’s employed clinical teaching faculty supervising Program 
Participants at MCDPH sites (“MCDPH Clinical Teaching Faculty”) to 
help them understand how to teach, evaluate, and optimize Program 
Participant performance. An accrediting body of the Global MD Program 
is the Australian Medical Council.  
 
2. In addition, Global MD’s Associate Dean for Faculty Affairs will identify 
and prioritize instructional development needs of MCDPH Clinical 
Teaching Faculty for the Global MD Program.  Other deliverables provided 
by the Global MD Program will include the development and 
implementation of instructional methods including assisting MCDPH 
Clinical 
Teaching 
Faculty 
in 
integrating 
technology 
across 
teaching/learning experiences, recruitment of MCDPH Clinical Teaching 
Faculty for inclusion in program development, implementation and 
development of resource and materials for MCDPH Clinical Teaching 
Faculty on a continual basis to guide teaching and/or assessment 
practices.  
 
3. MCDPH Clinical Teaching Faculty with faculty appointments must 
complete faculty development learning modules which will cover all 
aspects of training from greeting and orientation through assessment and 
examination. Learning modules will provide MCDPH Clinical Teaching 
Faculty with continuing medical education credits.  
 
B. Minimum Time Requirements  
1. Effective clinical teaching requires dedicated time and a planned 
balance between clinical care/service and teaching. MCDPH 
Clinical Teaching Faculty will have dedicated time to assure optimal 
Program Participant learning.  
 
2. Global MD Clerkship Directors will collaborate with site specific 
MCDPH Clinical Teaching Faculty to not only recruit high quality 
MCDPH Clinical Teaching Faculty, but also assure the quality time 
commitment is in place.

C. Maintenance of University Standards 
 
1. Global MD Leadership will complete clinical site visits at MCDPH site at a 
minimum annually. MCDPH clinical sites with new Clinical Teaching 
Faculty may expect quarterly visits to assess progress, needs and assure 
needed resources are available.  
 
2. Global MD will oversee the management and collection of academic data 
essential for academic planning, program assessment, and program 
improvement to include but not limited to the following:  
 
(a) Regular, online surveys will be administered to Program 
Participants and MCDPH Clinical Teaching Faculty at the 
conclusion of each education unit or clerkship.  
 
(b) Findings from these surveys will be communicated quarterly 
to the clerkship coordinators and clerkship directors by the 
Director of Assessment and Evaluation and presented to the 
Clinical Curriculum Committee (“CCC”). In this way, Global 
MD faculty and student representatives will receive the 
feedback of program and curricular evaluation and can 
partner with the Global MD Curricular Affairs leadership to 
affect necessary improvements or modifications of the 
curriculum to meet the needs of the Program Participants and 
MCDPH Clinical Teaching Faculty.  
 
(c) Additional expected sources of input regarding curriculum 
evaluation will include Program Participant focus groups 
conducted by the Director of Assessment and Evaluation and 
regular directed communication with the Program Participants 
via their Student Affairs leadership and Program Participant 
mentors.  
  
(d) Further, a confidential process will be deployed wherein 
Program Participants can anonymously report concerns to the 
Global 
MD 
Curricular 
Affairs 
leadership. 
Information 
generated by all such means will flow directly back to the 
Global MD Executive and Global MD CCC, along with survey 
findings to drive required improvements or modifications of 
the curriculum.  
 
3. The Health Service Provider agrees:  
 
(a) To use all reasonable endeavors to ensure that for the duration of 
the Agreement, Program Participants act only under the supervision 
of a suitably experienced, qualified, and (where relevant) registered 
clinician at all times when participating in the Global MD Program;

(b) That discipline of Program Participants is the responsibility of Global 
MD Program subject to the right of the Health Service Provider to 
instruct a Program Participant in connection with any clinical learning 
experience arising in the course of the Global MD Program;  
 
(c) To notify the Global MD Clerkship Director if a complaint is received 
by the Health Service Provider in relation to a Program Participant or 
a member of the Global MD staff regarding the Global MD Program;  
 
(d) To provide the facilities, subject to the operational requirement of the 
Health Service Provider, to enable Global MD staff to conduct clinical 
education discussion with the Program Participants; and  
 
(e) At the discretion of the Health Service Provider to provide (at no cost) 
Program Participant with protective garments for hygiene and 
infection control for use during the Global MD Program.  
 
D. Appointment of Faculty to the Medical Staff  
When appropriate as determined by the Global MD Program, certain faculty 
at clerkship sites may become faculty in the College of Health Sciences. This 
may come with appointment and fractional FTE in the Global MD Program.  
 
E. Design of the Curriculum  
1. The Global MD Program shall be responsible for:  
 
(a) The administration and development of the education program course 
content;  
 
(b) Clinical and practical programs;  
 
(c) The format of the education program’s Program Participant 
assessment;  
 
(d) Taking reasonable steps to inform the Health Service Provider of the 
specific learning objectives for Program Participants during their 
participation in the Global MD Program;  
 
(e) Taking reasonable steps to inform the Health Service Provider of the 
extent and nature of prior experience of Program Participants as 
relevant to the Global MD Program; and  
 
(f) Taking reasonable steps to inform the Health Service Provider of 
specific matters which may affect a Program Participant’s learning 
capacity as relevant to the Global MD Program and all related matters.  
 
F. Evaluation of Program Participant Performance

1. The Global MD Program shall be responsible for Program Participant 
assessment, unless it is otherwise agreed in writing by the Parties that 
the content of placement and assessment is to be conducted in 
collaboration with the Health Service Provider employees.  
 
2. Oversight of Program Participants will be provided by the MCDPH 
Clinical Teaching Faculty assigned to a particular Program Participant. 
The MCDPH Clinical Teaching Faculty will be assigned one (1) Program 
Participant per rotation. Program Participant assessment will be 
provided by that Affiliate Clinical Teaching Faculty and Program 
Participants have the opportunity to provide feedback (via either 
identified or anonymous mechanisms) on the Affiliate Clinical Teaching 
Faculty, as well as the facility and staff.  
 
III. 
These Special Provisions are not intended to limit the Parties to this Agreement 
from developing Special Provisions for other programs covered by this 
Agreement.  
 
IV. 
If a conflict arises between these Special Provisions and the General Provisions 
of this Agreement, the General Provisions shall govern.  
 
V. 
Section and clause headings are used herein solely for convenience of reference 
and are not intended as substantive parts of the Parties’ agreement.

Special Provisions 
 
I. 
These Special Provisions are intended to cover, but not be limited to, the training 
of Physician Assistant Program students (“Program Participants”) from the 
College of Health Sciences, University of Arizona while they are at MCDPH. 
 
II. 
Items of Agreement 
 
A. 
The Physician Assistant Program Director is responsible for the general 
administration of the program, including those activities related to the 
recruitment, selection, instruction, supervision, counseling, evaluation, 
discipline and advancement of Program Participants and the maintenance 
of records related to program accreditation.  
 
B. 
Supervisory MCDPH Staff (on-site). Preceptors shall include Physicians 
who are specialty board certified in their area of instruction, NCCPA certified 
physician assistants, and/or Other licensed health care providers qualified 
in their area of instruction including but not limited to: nurse practitioners, 
certified nurse midwives, clinical social workers, and psychologists 
(“Preceptors”).  
 
C. 
Educational Goals and Objectives. Course syllabus and learning objectives 
shall be provided to each Preceptor prior to the start of each clinical rotation. 
Program Participants shall have access to the physical facilities, patient 
populations, and clinical supervision necessary for fulfilling program 
learning outcomes.  
 
D. 
Assignment of Program Participants. Program Participants will be assigned 
to a Preceptor in accordance with the Preceptor’s preferences and 
availability.  
 
E. 
Program Participant Evaluation, Supervision and Teaching. The Physician 
Assistant Program and Preceptors shall have shared responsibility for the 
evaluation and provision of feedback for Program Participants per ARC-PA 
requirements. MCDPH agrees to comply with initial and ongoing evaluation 
of clinical sites and Preceptors in accordance with accreditation standards. 
Program Participants shall have access to educational resources and 
supervised clinical training experiences inclusive of the opportunity to (a) 
perform supervised comprehensive patient assessments, (b) participate in 
patient care decision making, and (c) assist in the development a plan for 
patient management. For clarity, MCDPH, together with its Preceptor, 
agrees to maintain exclusive control over MCDPH's patients’ care and be 
the responsible party for all decisions related to such health care and 
treatment.
