Journey The Road Ahead Group Home 1. Proof of Ownership (title deed or grant deed) for corporations/LLC EIN documents. 2. Narrative A detailed narrative is required to be considered for this application. The narrative must include: ➢ Type of residential care/group home (i.e., assisted living, behavioral health, etc.) • Assisted Living Group Home Journey The Road Ahead is an adult disabled, neurological Support Group Home Interior: This 3-bedroom home is equipped with TV’s, phones, computers along with personalized furniture for visual appeal and comfort. Exterior: Large front and back yard landscaped beautifully for activities with a Gazebo to be built, pavers or concrete to lead the way for wheelchair/walker accessibility. There will be weekly group activities, meetings, socializing for inspirational purposes held, and special meals prepared. This group home focuses on disabled patients with a neurological disorder that ranges from Multiple Sclerosis, Ataxia, Parkinson’s disease, etc. The home will be equipped with 6 available beds, 2 patients to a room, and each patient will have their Failed BY DATE Conditionally Approved Approved ASchwenner 06/10/2024 City of Glendale Development Services Department Planning Division own caretaker around the clock to support the staff. The support includes personal home care for the patient (feeding, bathing, cleaning, washing of linens and clothes, grocery shopping, medication support, and 3 square meals a day). The patient’s expenses could potentially be covered via AHCCS for all-inclusive package depending on the kind of coverage they have. AHCCCS (Arizona Health Care Cost Containment System) is Arizona’s Medicaid Managed Care Program. Mercy Care is contracted with AHCCCS to provide Medicaid covered benefits and services to Mercy Care members. Each patient will have the benefit support of Medicaid who will provide and engage the appropriate organizations for the following positions: Case worker – Each patient will receive an assigned case worker by Mercy care. The case worker will engage POP Health and Pallative Care for the following specialists: • Nurse Practitioner • LPN • Wound Care specialist- as needed • Speech Therapist specialist- as needed • Physical Therapist specialist- as needed ➢ Provide an Emergency Contact List which must include the owner, site manager, and person(s) not living at the group home. • Sharine Lowe-Moore • Sharnell Lowe • Daraka Gillespie • Margie Stewart ➢ A description of the residents being cared for (i.e., youth, seniors, women, veterans, do they have disabilities or other special needs?) This group home focuses on disabled patients with a neurological disorder from Multiple Sclerosis, Ataxia, Parkinson’s disease, etc. • Women & Veterans ➢ Floor Plan including emergency evacuation plan with exits marked • The floor plan has 5 emergency exits and 5 entry/exits total in the home. All emergency exits are located via the covered and uncovered patio doors. 1) The front door entry and exit 2) The Living Room has a sliding door exit which leads to the patio that joins the master bedroom suite on the side of the home. 3) The master bedroom has a sliding door exit which leads to the patio that joins the living room on the side of the home. 4) The kitchen has 2 exits (via the sliding door and via the garage door which both lead to the back yard) The exit that leads through the garage can also access the front yard or the back yard. ➢ Number of residents receiving care and/or not receiving care (family and/or staff) • A total of 6 patients are able to receive care in the group home. We will expand as opportunities allow. ➢ Number of staff not residing at the property • Approximately 6 staff members will not be residing at the property. • 1- 2 nurses on staff at night in rotation ➢ Number of residents able to drive, and number & location of onsite parking spaces. • Residents may not be able to drive themselves but will have access to transportation. There are 4 parking spaces however we will have an onsite van to transport patients as needed and also utilize AHCCCS Transportation NEMT (Non- Medical Emergency Transportation) • NEMT includes Veyo, Netcor, etc ➢ Overview of each resident's daily schedule/activities • Breakfast - 7am – 10am • Social Time / Free Style Fun Time - 11am - Physical activity, Games, Arts and Crafts, Movie Day, Music therapy, Store Run • Lunch - noon – 2pm • Scheduled appointments/Meetings • Dinner- 4pm – 7pm • Visiting hours over ➢ Explain how the facility will meet the standards outlined in the UDC Sections 35.3.102F Journey The Road Ahead is an assisted living residential group home that focuses on disabled patients with a neurological disorder that ranges from Multiple Sclerosis, Ataxia, Parkinson’s disease, etc. The home will be equipped with 6 available beds, 2 patients in a room. A total of 6 patients can receive care in the group home. We will expand up to 10 as opportunities allow.
