CALL TO ORDER THE LOCAL FIRE BOARD
Mayor Jerry Weiers called the meeting to order at 9:30 a.m.

APPROVAL OF THE MINUTES

minutes of August 27, 2025.

NEW BUSINESS
There was no new business.
 
6.
EXECUTIVE SESSION
Upon a public majority vote of a quorum of the Local Police Board, the Board may hold an
executive session, which will not be open to the public, regarding any item listed on the
agenda but only for the following purposes:
(i) discussion or consideration of personnel matters (A.R.S. § 38-431.03(A)(1));
(ii) discussion or consideration of records exempt by law from public inspection (A.R.S. §
38-431.03(A)(2));
(iii) discussion or consultation for legal advice with the city's attorneys (A.R.S. §
38-431.03(A)(3));
 
 
 
The Board voted to go into Executive Session at 9:31 a.m.  All attendees that were not
Board Members or supporting staff exited the conference room prior to the start of
Executive Session.

The Board reviewed and discussed personnel matters in consultation with the Board's
attorney.

ADJOURN EXECUTIVE SESSION AND RECONVENE IN OPEN SESSION
The Board adjourned Executive Session at 9:43 a.m. and reconvened in Open Session. 
Attendees who had exited the conference room were invited back in.
 
8.
CONSIDERATION AND POSSIBLE ACTION ON BOARD ITEMS
 
A.
Consideration and possible action on the Application for Line of Duty Death Survivor Benefits
submitted by Michele Sine, surviving spouse of deceased Fire Fighter Max Sine.
 
 
The Board reviewed the forensic pathologist’s report and engaged in a thorough discussion
regarding the circumstances surrounding the death of retired Fire Fighter Max Sine, who
passed away several years after his retirement. The report concluded that his medical
condition was causally related to exposures sustained in the line of duty during his
employment with the City. After careful consideration of the findings and applicable
provisions under the Public Safety Personnel Retirement System, the Board determined
that the evidence supports a line-of-duty connection. Accordingly, the Board voted to
approve Line of Duty Death Survivor Benefits for Michele Sine, surviving spouse of Fire
Fighter Max Sine.

B.
Consideration and possible action on the Accidental Disability Retirement and medical
records submitted for Fire Engineer Alexandria Matthews.
 
 
The Board reviewed the Independent Medical Evaluation (IME) and engaged in discussion
regarding the statutory questions. Following review and deliberation, the Board reached
agreement that Fire Engineer Alexandria Matthews meets the statutory criteria for an
Accidental Disability Retirement. Based on the findings of the Independent Medical
Evaluation and the determination that her medical condition is the result of a
workplace-related injury, the Board voted to approve the Accidental Disability Retirement
for Fire Engineer Alexandria Matthews.

Board will be held on November 19, 2025, at 9:30 a.m., at the Glendale Regional Public
Safety Training Center (GRPSTC) in the 1st Floor Conference Room 103, located at 11550
W. Glendale Ave., Glendale, Arizona, 85307.
