Appendix B Maricopa County Health Department Unidos Statement of Work 2019-2023 The Maricopa County Health Department is located in the fastest growing county in the US, adding more than 222 persons per day in 2016, with more than 4.2 million residents. Home to nine out of ten of the state’s largest cities including Phoenix (the fifth largest city in the U.S.), the county is the third largest public health jurisdiction in the country. Maricopa CHD provides regional leadership and fiscally responsible, necessary public services so that residents can enjoy living in a heathy and safe environment. Maricopa CHD has three departments focused on upstream public health approaches: Community Transformation which includes worksite health innovation, school wellness, and community health innovation and Community Health Action which covers WIC, family health and oral health. Maricopa County Public Health Services was selected as a partner for the AzPRC Core Research Project based on the criteria: 1) is a county health departments responsible for working with ADHS in administering public health and chronic disease prevention programs; 2) is located in a county with a large Mexican origin population which is the focus of the study. 3) Is located in a county in which a CHC had agreed to participate in the research and provide clinical data; 4) worked collaboratively to develop the research project; and 5) expressed interest in partnering in the research. The SOW for the Maricopa County Health Department is; Participate in the AzPRC CAB Research Committee that will meet monthly and be responsible for guiding overall research and interpreting and disseminating research outcomes; Work collaboratively with the CAB research committee and AzPRC research team to refine Unidos, a CHW community-clinical linkage model that will provide social support to individuals at risk for chronic disease and help them address the social determinants of health; Engage in planning activities to solidify our capacity to implement the community clinical linkage intervention with an FQHC; Beginning in year 3, employ a CHW or equivalent position trained in CHW Core Competencies to implement the community program; Participate in CHW training related to the research intervention; Collect survey data relevant to the community program being implemented by CHWs in the community; Participate in qualitative data collection activities such as focus groups with staff and group meetings; and Participate in dissemination and scale up activities as appropriate. Mr. Gallegos will be responsible for the overall coordination and supervision of all aspects of the study. This includes hiring, training, and supervising staff/students; recruiting study participants; coordinating treatment and assessment components; scheduling and staff assignments; and data management. In addition, he will conduct the orientation sessions, assist with statistical analyses, and be responsible for reporting the study's findings. Other Personnel Maria Ballon, Program Coordinator (26.38% FTE). Ms. Ballon will ensure effective program management and provide coordination of the training of community health worker in support of this project. Fringe Benefits Variable benefit rate of 19.45% of total salaries for Medicare, social security, unemployment, worker's compensation, long term disability, life insurance, and Arizona State Retirement System. Fixed benefits are calculated at $11,856.00 per 1 FTE x .07 FTE (years 1-2); x 0.27 FI'Es (years 3-5) for medical and dental coverage. Consultant ACRO - Temporary Services. Community Health Worker (CHW) teaches community members that they need to get screened for high blood pressure and cholesterol. Most of the time, people at risk do not feel sick and are not aware they have these conditions. CHW will encourage community members to ask their doctor what their goals should be for blood pressure and cholesterol. CHW teaches community members that uncontrolled high blood pressure and cholesterol can damage their eyes, kidneys, heart, blood vessels, and brain. High blood pressure can also lead to chronic kidney failure requiring dialysis. CHW will introduce community members to social workers and others who can help them apply for programs and insurance that can help pay for health care. A part-time Community Health Worker with average pay rate of $19-20 providing support 25 hours/week for 12 months for years 3-4. Travel In-state travel has been budgeted at $975 in years 4. All travel is in compliance with the state guidelines. Travel to Tucson of the Principal Investigator relating to consortium meetings in support of project, including: Mileage - $0-445 per mile x 250 miles/quarter x 4 meetings= $445/ 12-month grant cycle (years 1-5) Meals - $59 per day x 4 meetings= $236/ 12-month grant cycle (years 1-5) Mileage This line item covers the community health worker and/or program coordinator travel related to clinical linkage at a healthcare center and community outreach. The cost is calculated as: $0-445 per mile x 55 miles/month x 1 FTE = $294/ 12-month grant cycle (years 3-5) Other Direct Costs Participant Incentives. Incentives are included for 100 participants at $50 for compensation for the time in filling out the emotional wellness survey. Indirect Costs Maricopa County Department of Public Health approved indirect rate for FY 2022-23 is 18.9%. Departmental Indirect Rates are reestablished at the beginning of each fiscal year and the future Indirect Rate will be collected at the corresponding rates.
