REQUIRED RESPONSE BY: 08/10/2022

This correspondence is from the Check Enforcement Program at the Maricopa County
Attorney’s Office (MCAO). Arizona law allows the MCAO to collect restitution from individuals
who write bad checks and to use the collected funds to compensate the victims of the bad
checks.

Our records show that you or your business, or someone representing you or your business
previously submitted a claim to the MCAO Check Enforcement Program for collection purposes.
The MCAO Check Enforcement Program subsequently received funds from the fraudulent party
and our office mailed a restitution check or checks to the person (or entity) and address listed
as the payee above. Unfortunately, the check(s} came back to our office either as undeliverable
or deemed uncashed. The attached Summary of Unclaimed Funds summarizes the funds that
remain unclaimed.

if you are the legal claimant and would like to apply for these funds, please complete the
enclosed affidavit and follow the instructions provided therein. lf the payee is a business or
other entity, the representative of that entity requesting the funds must review the Note in the
affidavit instructions as additional documentation must be provided to verify claimant’s right to
the funds. The affidavit must be signed in the presence of a Notary Public and returned by mail
by the required response date noted above.

If we have not received a response by the required response date, the funds described in this
letter will be transmitted to the County Treasurer for deposit in the county bad check trust fund
for.use in administering the Check Enforcement Program, per A.R.S. § 13-1811(C}.

The payee, as noted above in-bold, is the official victim of record te which our office will issue
the funds. Once we have received your request and have verified your claim to the funds, a
restitution check will be issued in the name of the payee. Our office cannot issue the check in
an alternative manner. It is typical for this process to take twelve (12) weeks from the required
response date.
