AMENDMENT No. 3

MEMORANDUM OF UNDERSTANDING 
Between  
BANNER UNIVERSITY HEALTH PLANS 
& 
MARICOPA COUNTY, ARIZONA 
WHEREAS, Maricopa County (“County”) and Banner University Health Plans (“Contractor”) have entered into an 
Agreement for the purpose of establishing collaborative protocol executed June 12th, 2019 (“Agreement”).  
WHEREAS, County and Contractor have agreed to renew this Agreement. 
NOW, THEREFORE, the parties hereby agree to the following provision: 
The Agreement shall be renewed for a term from June 12th, 2022 and ending June 11th, 2024. 
ALL OTHER TERMS AND CONDITION REMAIN UNCHANGED. If there is any conflict between the 
terms of the Agreement and this Amendment, in all such events the terms of this Amendment shall 
control. 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both
