ARIZONA PHYSICIANS IPA, INC. D/B/A UNITEDHEALTHCARE COMMUNITY PLAN 
& 
MARICOPA COUNTY, ARIZONA 
 
WHEREAS, Maricopa County (“County”) and Arizona Physicians IPA, Inc. d/b/a UnitedHealthcare Community Plan 
(“Contractor”) have entered into an Agreement for the purpose of establishing collaborative protocol executed December 11th, 
2019 (“Agreement”). 
 
WHEREAS, County and Contractor have agreed to modify the Agreement by changing certain terms and conditions;  
 
NOW, THEREFORE, the parties hereby agree to the following provisions: 
 
1. 
The Agreement shall be retroactively renewed for a term from December 12th, 2021 and ending December 11th, 
2022.  
 
ALL OTHER TERMS AND CONDITION REMAIN UNCHANGED. If there is any conflict between the terms of the 
Agreement and this Amendment, in all such events the terms of this Amendment shall control. 
 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
 
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both Parties
