MEMORANDUM OF UNDERSTANDING

MOLINA HEALTHCARE OF ARIZONA 
& 
MARICOPA COUNTY, ARIZONA 
 
WHEREAS, Maricopa County (“County”) and Magellan Complete Care of Arizona (“Contractor”) have entered into an 
Agreement for the purpose of establishing collaborative protocol executed July 31st, 2019 (“Agreement”). 
 
WHEREAS, County and Contractor have agreed to renew this Agreement; and 
 
WHEREAS, Contractor has changes it name to Molina Healthcare of Arizona. 
 
NOW, THEREFORE, the parties hereby agree to the following provision: 
 
1. 
The Agreement shall be renewed for a term from July 31st, 2022 and ending July 30th, 2023.  
 
ALL OTHER TERMS AND CONDITION REMAIN UNCHANGED. If there is any conflict between the terms of 
the Agreement and this Amendment, in all such events the terms of this Amendment shall control. 
 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
 
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both
