Dear Ms. Drenth
The Arizona Department of Health Services, High Risk Perinatal Program is pleased to contract with your 
agency to provide Community Nursing Services, through contract number CTR046941 for fiscal year 2023.  
Funding for this program is “as needed” based on client visits/enrollments.  Each year a “not to exceed” limit 
is budgeted for the contract.  This allows the Arizona Department of Health Services greater flexibility in 
allocating funds; based on the services being provided, without the need for a contract amendment should 
adjustments be required.  The “not to exceed” amount is subject to increase or decrease during the fiscal year 
based on contractor performance, availability of funds, and program needs.  Any deviation to the “not to 
exceed” amounts must receive prior approval from the ADHS Home Visiting Program Manager.
The contract period is from July 1, 2022 to June 30, 2023.  The contract should be utilized “as needed” but 
should “not exceed” the following allocated amounts:
