AMENDMENT NO. 4 
TO THE CONTRACT 
BETWEEN 
MARICOPA COUNTY 
ADMINISTERED BY ITS 
HUMAN SERVICES DEPARTMENT 
AND 
DIOCESAN COUNCIL FOR THE SOCIETY OF SAINT VINCENT DE PAUL 
 
 
I. 
Maricopa County (“County”), administered by its Human Services Department, and 
Diocesan Council for the Society of Saint Vincent de Paul, a non-profit, community-based 
organization (“Contractor”) entered into a financial agreement (“Agreement”), which was 
fully executed on or about June 16, 2021, for weather relief accommodations for people 
experiencing homelessness. The Agreement Start Date is June 9, 2021, and the 
Termination Date is September 30, 2021. In accordance with the terms of the Agreement, 
the County will pay the Contractor an amount not to exceed $387,782 for those services. 
The County and Contractor collectively are referred to as the “Parties.” 
 
The Parties executed Amendment No. 1 on or about August 4, 2021 to increase funding 
under the agreement to $731,464. The Parties executed Amendment No. 2 on or about 
September 1, 2021 to increase funding under the agreement to $808,503. 
 
The Parties executed Amendment No. 3 on or about April 9, 2022, to extend the 
Agreement to June 30, 2023. In addition, it added a Work Statement for “Heat Relief” and 
provided the Contractor with $2,003,920 in Assistance Listing Number (ALN) 81.135 
American Rescue Plan Act (ARPA) funds for the Heat Relief Work Statement services. 
Finally, and provided $88,845 in General Funds through October 31, 2022, for drinking 
water. 
 
II. 
The Parties agree to enter into this Amendment No. 4 to amend the Agreement as follows: 
 
A. 
The County shall provide Contractor with $96,090 in County General Funds for 
Janitorial Services. The funding availability is through October 31, 2022. The 
monthly janitorial services are anticipated to be $16,015. 
 
B. 
Add that the County is updating requirements whereby Saint Vincent de Paul and 
all workers providing services in this agreement will be subject to background and 
security checks and screening. Details regarding Background Screenings are 
attached as Attachment A. The Subrecipient shall comply with all applicable 
County laws, rules, and regulations. 
 
C. 
The Agreement funding amount shall be increased from $2,901,268 to $2,997,358. 
 
III. 
Section II above contains all the changes made by this Amendment No. 4. All other terms 
and conditions of the Agreement and previously approved Amendments shall remain the 
same and in full force and effect as approved and amended. 
 
IV. 
The Parties have authorized the undersigned to execute this Amendment No. 4 on their 
behalf, and it shall be effective upon approval and signature by both Parties.

Attachment A 
Background Screening Details 
 
 
1.0 
Background Screening Risk Level 
The County has established a Maximum Risk Level. 
 
2.0 
Maximum Risk Level 
A maximum risk background screening will be performed every five years when the 
Contract Worker’s work assignment will: 
2.1 
Work directly with vulnerable adults or children (under age 18); or 
2.2 
Any responsibility for the receipt of payment of County funds of control of 
inventories, assets, or records that are at risk of misappropriation; or 
2.3 
Unescorted access to: 
2.3.1 County data centers, money rooms, high-value equipment rooms; or 
2.3.2 Unescorted access to private residences; or 
2.3.3 Access to critical infrastructure sites/facilities; or 
2.3.4 Direct or remote access to Criminal Justice Information Systems (CJIS) 
infrastructure. 
 
3.0 
Requirements 
The background screening for maximum risk level will include a background check for real 
identity/legal name and will include felony and misdemeanor records from any county in 
the United States, the State of Arizona, plus any other jurisdiction where the Contractor 
worker has lived at any time in the preceding seven years from the Contractor Worker’s 
proposed date of hire. In addition, Maximum screening levels may require additional 
checks as included herein, depending on the scope of work, and may be amended if the 
scope of work changes. 
 
4.0 
Additional Maximum Risk Background Checks 
Maximum screening will additionally require: 
4.1 
Credit Check (for cash handling, accounting, and compliance positions only); 
4.2 
Driving records (for driving positions only); or 
4.3 
Fingerprint verification (only for supervisors of Contract Workers working directly 
with children or vulnerable adults or scope takes the individual to a County location 
with Criminal Justice Information System (CJIS) access. 
 
5.0 
Continuing Duty; Audit 
Saint Vincent de Paul will maintain all records and documents related to all background 
screenings and the County reserves the right to audit Saint Vincent de Paul’s records.
