I honestly was really surprised to hear you made the hike to downtown finance back in March. The challenge is you created your own affidavit note and had it notarized. Finance gave you the forms they needed you notarize, you cannot make your own. I’ve attached the forms to this email that need to be notarized. You also need to create a document that you are the President of your company and have permission to sign the documentation. There is a sample provided and instructions in the attachment. The affidavit and permission letter need to be signed and sent hard copy to our Department of Finance. If you want to deliver them again, you are welcome too however, I recommend you send them in the mail. We receive them in a pretty timely manner. If you have any questions, please reach out. Please check with the office staff again, if you cannot find it I will go notarized again. The reason I hand deliver to your office is because I worry the only hard copy may be lost in the mail. Upon further investigation, this is what I was able to find. As of 6/23/22 we know this check has not been cashed. Additionally, We have been in touch with your landlord since February. We have asked him to complete the affidavit forms to have the payment reissued. He did not do that. He made his own affidavit form which wasn’t acceptable for the County. Mr. Shieh actually came down to the Maricopa County Finance Department on 3/18/22 and they helped him by providing him with the forms we needed to be completed. He never returned them to us and we need them via hardcopy. He can have the attached document notarized and provide the letter stating he can sign on behalf of his company. He is not required to bring them to us; he can send these forms to us via the Postal Service at the following address: Maricopa County Department of Finance Attn: Joyce Maurmann Unless the affidavit forms and required authorization letter is completed we cannot move forward in having the check re-issued. I would recommend contacting your landlord if you want any action taken on the $3,000. He is elderly and may need assistance. Perhaps, you can help him.
