I.
The above-named contract is hereby amended as specified below:
A. Sections have been added under Exhibit C, 1.1 PURPOSE OF THE PROGRAM, 
AUTHORITY AND BACKGROUND as shown on Page 2.
II.
All other terms and conditions of the original contract shall remain in full force and 
effect.

AMENDMENT 2 – ADDED SECTIONS
Exhibit C - 1.1, PURPOSE OF THE PROGRAM, AUTHORITY AND BACKGROUND
1.1.1
Contractor shall utilize funds being received from MCDPH for the purchase and 
implementation of an Electronic Health Record (EHR) service as chosen and 
selected by the Contractor. The estimated total monthly recurring fee is $1,380 
plus a one-time implementation fee of $45,000 for a total amount for the first year 
of $61,560. These are considered administrative funds and are not eligible for 
indirect to be charged. Funding for this amended service is provided by the 
‘Ending the HIV Epidemic’ grant and the contingency of this service is subject to 
the availability of appropriated funds. 
1.1.2
Contractor shall repay any portion of the funds not used for the specified purpose. 
Contractor will work directly with an Electronic Health Record service provider 
to set up implementation, virtual training, and payment of services rendered. On-
site training costs are not an allowable use of these funds; therefore, Contractor 
shall use alternate funds if they choose to have on-site training.   
1.1.2.1 The selected Electronic Health Record system must include the following 
specifications at a minimum: Scheduling and Intake; Financial Clearance 
and Eligibility; Clinical Documentation; Assessments and Treatment 
Planning; Orders and Results; Med Administration and Management; 
ePrescribing; Billing and Collections; Reporting/Analytics; Compliance 
and Audit Management.
