THE ARIZONA ADVERSE CHILDHOOD EXPERIENCES CONSORTIUM
for
COMMUNITY SUPPORT FOR ADVERSE CHILDHOOD EXPERIENCES
I.
The above-named contract is hereby amended as specified below:
A. Section 1.1, CONTRACT TERM, is changed to reflect a new expiration date of December 31, 2024.
B. Exhibit A, Pricing Sheet, TOTAL BUDGET REQUESTED, is changed to $3,333,588. The
additional $1,333,588 shall be used to extend the contract period one year as well as expand certain
services already being provided in the current Scope of Work.
C. Exhibit B, SCOPE OF WORK, shall be amended to include details regarding expansion of the
Adverse Childhood Training Program, an increase in the number of Youth Mental Health First
Trainings to be given, and to identify the site chosen for the Youth Program. This addition to the scope
of work begins on Page 2 of this amendment.
II.
All other terms and conditions of the original contract shall remain in full force and effect.

EXHIBIT B, 2.0 SCOPE OF WORK CHANGES/ADDITIONS
2.1
A.
Increase the number of trauma awareness school-based training programs from 300 to 480, an increase of
180, to be completed by December 31, 2024.
B.
Increase the number of youth trauma education programs from 180 to 280, an increase of 100 to be
completed by December 31, 2024.
C.
Increase the number of single-session administrative trauma education programs from 150 to 200, an
increase of 50 to be completed by December 31, 2024.
D.
Increase the number of community organization trauma awareness education programs from 20 to 35, an
increase of 15 to be completed by December 31, 2024.
E.
The number of locations the above trainings shall take place at shall increase from 300 to 420, an increase of
120 sites.
F.
The Phoenix Children’s Hospital/Center for Resiliency and Wellbeing has been selected as the site to host
the Youth Program.
G.
The number of Youth Mental Health First Trainings shall increase.
