INVOCATION - INVOCACIÓN
Supervisor Gallardo introduced Daniel Valenzuela, former City of Phoenix councilman, 
who offered the invocation.
2.
PLEDGE OF ALLEGIANCE - JURO FIDELIDAD A LA BANDERA
Daniel Valenzuela led the assemblage in the Pledge of Allegiance to the flag.
Supervisor Gallardo thanked Mr. Valenzuela for all he has done in the west valley, as 
a former City of Phoenix councilman and as a firefighter. He has most recently served 
on the panel for the Citizens’ Advisory Committee to fill the vacancy in the Senate for 
Legislative District 30. Supervisor Gallardo said he has been an excellent public servant 
in many regards.
Mr. Valenzuela thanked the Citizens’ panel for volunteering to serve and make a 
recommendation for the Board of Supervisors to appoint.
Supervisor Gates thanked Mr. Valenzuela and said he served with him on the Phoenix 
City Council and commented that he was highly respected by everyone on the council.

PET SHOWCASE BY MARICOPA COUNTY ANIMAL CARE AND CONTROL - 
PRESENTACIÓN DE ANIMALES DOMESTICOS POR EL DEPARTAMENTO DE 
CONTROL Y CUIDADO DE ANIMALES
Monica Gery introduced Garlic who has been waiting for 140 days for someone to turn 
his luck around. The staff at the shelter have taught him to walk on a leash and to be 
friendly with people. He has been neutered, microchipped, and vaccinated and the 
adoption fee for Garlic is $25 and is located at the East Shelter. For more information, 
visit pets.maricopa.gov.
Chairman Sellers asked the Clerk if there were any announcements or corrections to 
the agenda. The Clerk responded there were none.

APPOINTMENT FOR STATE SENATOR LEGISLATIVE DISTRICT 30
Pursuant to A.R.S. § 41-1202, receive the names of the three qualified electors as 
candidates to fill the vacancy for the Legislative District 30 State Senator seat as submitted

by the Legislative District 30 Citizens Panel and appoint one of the candidates from the list 
to fill the vacancy created by the resignation of State Senator Otoniel "Tony" Navarrete.
The three candidate names submitted by the Citizens Panel are:

Supervisor Gallardo commented that the Board of Supervisors take filling Legislative 
vacancies very seriously. He said this was a unique situation in that this Legislative 
District did not have 30 precinct committeemen who would normally provide the Board 
with recommendations to fill the vacancy. Because there were not 30 precinct 
committeemen, this responsibility fell to the Board of Supervisors who put together a 
Citizens’ Panel to provide three recommendations to the Board. He thanked the 
members of the Citizens’ Panel for serving and noted their attributes. He thanked Daniel 
Valenzuela for his leadership as chairman of this panel.  
Motion was made by Supervisor Steve Gallardo, seconded by Supervisor Bill Gates to 
appoint Raquel Teran to fill the vacancy for the Legislative District 30 Senate seat.
Supervisor Gates stated there is a long practice of respecting the wishes of the 
Supervisor whose district the legislative seat represents. He thanked Supervisor 
Gallardo for making an appointment that makes the best sense for his constituents.
Supervisor Hickman stated this responsibility is very important to the community. He 
said when making this appointment, the Board will consider the Supervisorial Districts 
impacted by the appointment. He said there were four Legislative District 30 precincts 
in District 4 and Supervisor Gates has eight Legislative District 30 precincts in his district 
three, and he supports his colleagues in this decision.

RECOGNITION OF BLAIR BRADSHAW, FINANCIAL SERVICES DIVISION DIRECTOR, 
TREASURER'S OFFICE
Recognition of Blair Bradshaw, Financial Services Division Director, for 30 years of dedicated 
service to Maricopa County. (C-06-22-867-X-00)
Chairman Sellers invited John Allen, County Treasurer, to come forward.
Mr. Allen noted happenings and events 30 years ago when Blair Bradshaw joined 
Maricopa County. He said Mr. Bradshaw has been an institutional person to the 
Treasurer’s Office. Since taking office, Mr. Allen stated Mr. Bradshaw had been a fount 
of knowledge, walking him through the history of the Treasurer’s Office. He was 
honored to introduce Mr. Blair Bradshaw.
Mr. Bradshaw stated it had been a pleasure to work for Maricopa County. He said it has 
been a privilege to work with the many other County entities and staff.  He has tried to 
assist the public in resolving their issues so they could move forward. He thanked the 
staff in the accounting division and the Treasurer’s Office as a whole. Mr. Bradshaw 
thanked the Board of Supervisors for their leadership through the years and appreciated 
the many great friendships he developed.

Supervisor Chucri commented that 30 years of service is not seen very often. He said 
he appreciated Mr. Bradshaw’s exit plan, sharing the history of the Treasurer’s Office 
with the current Treasurer. He acknowledged this is not easy to do. He wished Mr. 
Bradshaw well and thanked him for his years of service.

RECOGNITION OF BRUCE LIGGETT, DIRECTOR, HUMAN SERVICES
Recognition of Bruce Liggett, Human Services Director, for 8 years of dedicated service to 
Maricopa County. (C-06-22-868-X-00)
Chairman Sellers introduced Lee Ann Bohn, Assistant County Manager. She shared 
her favorite Bruce Liggett story with the Board. She said in 2018 she had the opportunity 
to travel around the County with Mr. Liggett’s assistant directors. She asked each one 
what brought them to work for the County and each one said they came to work for Mr. 
Liggett. She said he is a leader, friend, mentor, and she noted some of his 
accomplishments. Ms. Bohn credited Mr. Liggett with the Hand-in-Hand service that 
helps break the cycle of incarceration of homeless people. He and his workforce have 
created partnerships throughout the valley to provide services within their communities. 
Ms. Bohn noted in the last 18 months when it was realized how vulnerable the 
downtown homeless were to COVID, Mr. Liggett and his team set up safe outdoor 
spaces on County land, providing space for social distancing, clean water, restrooms, 
and handwashing stations. She said 820 people were served during the SOS Program 
and 255 people were transitioned to permanent housing. Together with his staff, they 
have helped over 14,000 people keep roofs over their heads during the pandemic and 
provided services to the 2,400 homeless that needed to be quarantined. He has 
brokered new relationships to support sexual, and domestic violence survivors in 
Maricopa County.  She recognized Mr. Liggett’s team as he would be the first to say he 
accomplished nothing on his own.  When she stepped into her current role three years 
ago, he was patient and provided her a history of the department and the context of the 
programs. She thanked him for giving the people of Maricopa County his passion and 
for being a friend to so many colleagues.
Chairman Sellers introduced a video in honor of Bruce Liggett.
Joy Rich, County Manager, said Mr. Liggett’s legacy could not be measured. She said 
his service is who he is, and he has impacted lives repeatedly. She stated she was 
proud to know him and to have worked with him. He taught her to be more measured 
and thoughtful. She said everyone who knows him loves him.
Chairman Sellers acknowledged Sheriff Paul Penzone being present to congratulate 
Mr. Liggett on his retirement.
Supervisor Chucri echoed the comments of Ms. Rich and Ms. Bohn and said you 
seldom see someone with the passion that Mr. Liggett brings to the job. He said the 
thought process Mr. Liggett puts into everything has made a remarkable difference in 
the lives he has touched. Mr. Liggett never grew tired of finding a better way to serving 
the community. Supervisor Chucri stated when he talks about Maricopa County being 
best in class, he was talking about Bruce Liggett.
Supervisor Gallardo acknowledged this as an opportunity to say wonderful things about 
Bruce Liggett. He said his hero was Jodie Liggett, Mr. Liggett’s wife and Bruce is her 
plus one. He noted that Mr. Liggett has turned so many lives around and he said when 
District 5 decided to spread the word regarding all the different programs provided and 
available, it moved the needle in the lives of his constituents. The pandemic elevated 
the work of the Human Services Department and the services that have been available

for many years. He stated Mr. Liggett would be missed and thanked him for all he has 
done for Maricopa County.
Supervisor Hickman shared a story about his mother-in-law who previously worked with 
the state as Director of Child Protective Services. He said eight years ago when he was 
appointed to the Board of Supervisors, he wanted to know about hires made for the 
County. He asked his mother-in-law about Mr. Liggett, then a state employee. She told 
Supervisor Hickman the Board “would not make a better hire” and it was so true. He 
said he had the pleasure of working with Mr. Liggett and his team each day through the 
pandemic. He said even in today’s meeting, there is a fourth quarter report from 
Workforce Development that shows Mr. Liggett has left Workforce Development in a 
better place than when he got to the County. Supervisor Hickman thanked Mr. Liggett 
for the many ways he has made Maricopa County better.
Supervisor Gates congratulated Mr. Liggett on his retirement and said Mr. Liggett could 
look back at his accomplishments and know he changed and saved lives. Supervisor 
Gates noted Mr. Liggett has a unique set of skills. There is a high level of compassion, 
combined with the ability to work both within the government and outside the 
government. He stated that Mr. Liggett and his team are the heart of Maricopa County. 
He noted all the programs Mr. Liggett has touched and how they made the Board more 
human. He said it has been a pleasure and a privilege to work with Mr. Liggett.
Chairman Sellers stated after listening to his colleagues, it is clear the impact Mr. Liggett 
has had on Maricopa County. He shared his experience of reading to the children at a 
Head Start program. He said this was an education for him on the impact this had not 
only on the children but also the parents. He said he admired Mr. Liggett’s leadership 
and his team and added that Mr. Liggett and his team have made Maricopa County 
very proud. 
Mr. Bruce Liggett came forward and said it was a privilege to work with Maricopa 
County. He said from the first day on the job, he felt respected as a Human Services 
professional. He said working under Joy Rich’s and Lee Ann Bohn’s leadership, the 
entire team has been very successful. He stated he is humbled by all his professional 
counterparts in the County that he has worked with. Mr. Liggett said he has been lucky 
in his career, and he noted several programs or services he was a part of that helped 
people in diverse walks of life. He said with COVID being the capstone of his career, it 
was time to retire. He said these are hard jobs, investing heart, mind, and emotions into 
the job, and it was time to pause and slow down. He spoke about his staff and informed 
the Board they were a good team to lead going forward. He said the highlight of his 
career has been working at Maricopa County. He spoke of his wife and her support of 
his career.
Chairman Sellers asked the County Manager to come forward for a picture with Mr. 
Liggett and the Board. Chairman Sellers presented him with a plaque and a clock for 
his service.

Information Inc. in the amount of $480,652 for the software licensing and use of Co-Star’s 
Commercial Real Estate application.
Co-Star Realty Information, Inc. provides a web-based application that allows our office to 
retrieve accurate property and sales data for commercial property within Maricopa County. 
This data is then used to assist us with our property valuation requirements.  This is the only 
vendor that is known who provides this type of data for all commercial property types.  Our 
office has used Co-Star for many years and have built our processes around Co-Star's web

AGREEMENT WITH US DEPARTMENT OF JUSTICE, DRUG ENFORCEMENT 
ADMINISTRATION FOR THE DEA TASK FORCE FFY 2022
Approve the Task Force Agreement and acceptance of any available reimbursement funding 
up to $19,370.00 from the US Department of Justice, Drug Enforcement Administration. The 
term of this agreement is October 1, 2021 through September 30, 2022. Authorize the 
Chairman to sign all documents related to these funds, as applicable. The agreement allows 
a 0% rate for indirect costs, or $0 which may be incurred by the County Attorney's Office or 
Maricopa County for the administration of this grant. The Maricopa County Attorney's Office's 
composite indirect cost rate is 20.3%, or $3,932. The recoverable indirect cost of 
administering this grant is $0; the non-recoverable indirect cost is $3,932. Grant revenues 
are not local revenues for the purpose of the constitutional expenditure limitation, and 
therefore, expenditure of the funds is not prohibited by the budget law. The agreement does 
not require on-going cash contributions after the period end date. This agreement is 
recurring, non-competitive and does not require a match. This funding indirectly supports the 
overall mandated function of prosecution of criminal cases.  Approval of this agreement 
allows for reimbursement of overtime paid to the Maricopa County Attorney’s Office detective 
assigned to the DEA Task Force. The purpose of the agreement is to enhance disruption of 
the illicit drug traffic in Maricopa County and the State of Arizona by immobilizing targeted 
trafficking violators and organizations. The County Attorney has cooperated with the DEA 
since 1998. While the agreement allows for the reimbursement of overtime, it is contingent 
upon the availability of funds. The County Attorney is prepared to absorb the costs of any

ADMINISTRATION FOR THE DEA PHOENIX TASK FORCE FFY2022
Approve the Agreement and acceptance of up to $38,740 from the U.S. Department of 
Justice, Drug Enforcement Administration for MCSO participation in the DEA Phoenix Task 
Forces. The term of this agreement is October 1, 2021 through September 30, 2022.  
The grant award is reoccurring and has been awarded to the Sheriff's Office since 1998. A 
cash or in-kind match is not applicable. There is no future or ongoing contribution required 
following termination of the grant period. The Sheriff’s indirect cost rate is 14.74% for a total 
of $5,710.28 none of which is recoverable per item 7 of the Agreement. The unrecoverable 
amount of $5,710.28 will be absorbed by the Sheriff’s Office General Fund. The grant award 
is not a mandated function but provides a benefit to the citizens by enhancing public safety 
through illegal drug interdiction. The grant award is not competitively bid.   
This agreement reimburses overtime costs for two deputies assigned to the Phoenix Task 
Force, up to $19,370 per deputy for a total of $38,740.  
These revenues are not local revenues for the purpose of the constitutional expenditure 
limitation, and therefore, expenditures of these revenues are not prohibited by the budget

AMENDMENT TO AGREEMENT FROM THE ARIZONA DEPARTMENT OF HOMELAND 
SECURITY, PROJECT #200812-05 VEHICLE MOUNTED RADIATION DETECTION
Approve Amendment #2 to the Subrecipient Agreement from the Arizona Department of 
Homeland Security, Project #200812-05. This amendment extends the expiration date of the 
Agreement from September 30, 2021 until September 30, 2022. All other terms and 
conditions are the same. This funding award is not recurring and there is no match 
requirement. It was competitively bid. The purpose for which it is being used it is not a 
mandated function.  
This Amendment extends the amount of time to purchase the Vehicle Mounted Radiation 
Detection and Identification System. This extension will allow MCSO to complete all 
purchases by 9/30/2022. The original award amount of $40,000 was approved on December

AGREEMENT WITH US ENVIRONMENTAL PROTECTION AGENCY TO MONITOR 
PM2.5
Approve an Agreement between the US Environmental Protection Agency (EPA) and 
Maricopa County through the Air Quality Department in the not-to-exceed amount of 
$123,245. This assistance is funded by a grant from the US Environmental Protection 
Agency, Award ID# PM-98T03501-1 which provides full federal funding in the amount of 
$123,245.
The Agreement is effective from April 1, 2021 until March 31, 2022, while the budget is 
retroactively effective from April 01, 2021 to March 31, 2022. Project budgets are applied 
annually. This agreement is subject to applicable EPA regulatory and statutory provisions, 
all terms and conditions of this agreement and any attachments. The FY2021 Air Quality 
composite indirect rate of 21.11% will be applied to the direct charges of $43,245. Indirect 
costs of $7,538 are fully recoverable. A capital equipment expense of $80,000 is included.
Grant revenues are not local revenues for the purpose of the constitutional expenditure 
limitation; therefore, expenditures of these revenues are not prohibited by the budget law. A

GOVERNMENTS FOR THE TRAVEL REDUCTION PROGRAM
Approve Amendment Two Agreement Number 1024 between the Maricopa Association of 
Governments (MAG) and Maricopa County (County) through the Air Quality Department 
(MCAQD) in the not-to-exceed amount of $962,347. The purpose of this amendment is to 
fund the Unified Planning Work Program and revise the scope of services. This amendment 
is effective from October 1, 2021 to September 30, 2022. All other terms and conditions of 
the Agreement remain in full force and effect.
The Agreement between MAG and MCAQD is reoccurring and has been approved by the 
Board of Supervisors in previous years. The cash or in-kind match is not applicable, indirect

cost is fully recoverable, ongoing cash contributions not applicable. The Agreement is not 
mandated, and the activities are 100% financially supported by the Agreement from MAG. 
The Agreement does not go through the bid process. There are no costs that will need to be 
absorbed by the MCAQD’s operating budget.
An amount of up to $356,000 will be passed through to the Regional Public Transportation 
Authority as an external service provider upon separate agenda and approval for services 
provided between October 1, 2021 and September 30, 2022. 
This item has been budgeted and approved by the Board of Supervisors with the adoption 
of the FY2022 MCAQD budget. Grant revenues are not local revenues for the purpose of 
the constitutional expenditure limitation, and therefore expenditures of these revenues are 
not prohibited by the budget law. This Agreement is funded with Federal Funds, 
CFDA#20.205.
This item was originally approved under C-85-20-005-G-00 by the Board of Supervisors on

MARICOPA COUNTY PROHIBITION OF SMOKING AND TOBACCO USE ON COUNTY 
PROPERTY POLICY (A1919) REVISIONS
Approve the revisions to the Maricopa County Prohibition of Smoking and Tobacco Use on 
County Property Policy (A1919) effective 9/15/2021. These revisions update policy language 
for consistency with the current policy format.
This policy applies to all employees of Maricopa County elected offices and appointed 
departments, as well as the Flood Control District of Maricopa County and the Maricopa 
County Library District (Special Districts). The Board of Supervisors is authorized to jointly 
adopt policies applying to the Special Districts under the Intergovernmental Agreement, C-
06-18-393-6-00, approved on April 11, 2018. This policy also applies to County volunteers, 
contractors, clients, and visitors while utilizing County buildings and adjacent areas, 
excluding County parklands.
This policy was originally approved in June of 1993 and later approved by the Board on May

AMEND PREMIUM PAY RATES
Approve the attached Maricopa County Premium Pay Rates effective September 6, 2021. 
Human Resources recommends adding Behavioral Health Technicians and Emergency 
Medical Technicians as eligible positions for the Correctional Health Shift Differentials. These 
two positions were recently created by Correctional Health.
On June 23, 2021, the Board approved revisions to the Compensation Policy, which included 
a sign-on incentive for registered nurses. Human Resources recommends moving the sign-
on incentive amounts and eligible positions to the Premium Pay Rates for consistency with 
other Board-approved rates. Human Resources also recommends a sign-on incentive for 
Detention Officers ($1,500 after passing the academy and $1,500 after 1 year).
These premium pay rates apply to the County’s elected offices, appointed departments, 
Judicial Branch of Maricopa County who receives funding from Maricopa County,  and the 
Flood Control District of Maricopa County and the Maricopa County Library District (Special 
Districts). The Board of Supervisors is authorized to jointly adopt policies applying to the 
Special Districts under the Intergovernmental Agreement, C-06-18-393-6-00, approved on

and Medical Authority (“Subrecipient”) and Maricopa County (“County”) administered by its 
Human Services Department.
The purpose of the Amendment is to address the following items:
A. Revise Section 3 (Work Statement) Paragraph 2.0 (Scope of Work) to address the 
following:
2.1 Project Description: add Whittman fire station to purchase and install a back-up 
generator.
2.3 Project Beneficiaries: add The Whittman Fire Station provides public health and safety 
to 3,007 housing units across 35-square miles. Both fire stations have approximately 20% of 
the service population with disabilities.
B. Increase the Agreement amount by $107,374 from $107,374 to $214,748 to purchase and 
install a back-up generator for the Whitman Arizona fire station.
C. Funding for Amendment No. 1 is provided by U.S. Department of Housing and Urban 
Development (HUD) Community Development Block Grant (CDBG) funds.

TEMPORARY HOMELESS SHELTER OPERATIONS
Approve financial Amendment No. 7 to the Agreement between Community Bridges, Inc. 
(Subrecipient), a community-based organization and Maricopa County (County) 
administered by its Human Services Department. The purpose of the Agreement is for the 
operation of temporary emergency shelter services for homeless individuals experiencing 
homelessness in Maricopa County in an effort to protect them from exposure to COVID-19. 
The Subrecipient and the County shall be referred to as the “Parties.”
The purpose of the Amendment is to address the following:
A. Extend the term of the Agreement from October 1, 2021 through December 31, 2021.
B. Add that the County shall increase funding by a not-to-exceed amount of $328,146. 
Funding availability period shall begin on October 1, 2021 through December 31, 2021. The

Approve Amendment No. 1 to the Intergovernmental Agreement (“Agreement”) between the 
Town of Gila Bend (“Subrecipient”) and Maricopa County (the “County”) administered by its 
Human Services Department.
The purpose of the Amendment is to address the following items:
A. The County shall provide the Subrecipient with $125,800 in U.S. Department of Housing 
and Urban Development (HUD) Coronavirus Aid, Relief, and Economic Security Act (CARES 
Act), Public Law 116-136, Community Development Block Grant Coronavirus (CDBG-CV) 
funding. The funding is to provide non-congregant shelter to 38 individuals and families who 
experienced substantial damage to their homes which are currently uninhabitable as a result 
of the August 14, 2021, flood in the Town of Gila Bend. Providing non-congregant shelter to 
individuals and families will help to prevent the spread of COVID-19 as the individuals and 
families will be able to live in separate spaces. Period of funding availability is August 14, 
2021 through June 30, 2023. 
B. Revised Section 4 (Budget and Compensation) Paragraph 1.0 (Budget) to increase 
funding from $563,082 to $688,882.

AMENDMENT TO GRANT FUNDS FROM U.S. DEPARTMENT OF HEALTH AND 
HUMAN SERVICES FOR HEAD START AND EARLY HEAD START PROGRAMS
Approve Amendment No. 1 to the grant funds from the U.S. Department of Health and 
Human Services (DHHS)/Administration for Children and Families (ACF), Office of Head 
Start (OHS), Grant No. 09CH012079-01-01. The Amendment will absorb grant 09HP000105 
into this grant, which will increase funding and child enrollment levels. 
The Amendment one-time is funding amounts increase the Early Head Start funding by 
$1,631,619 for a total of $7,662,116. The Amendment shall be effective on September 1, 
2021 through June 30, 2022. Funds are awarded to the County through a non-competitive 
process, since the County is a current recipient of Head Start grant funds.
The grant funding amount shall be revised to $19,438,445 for the Budget Period of July 1, 
2021 through June 30, 2022:
• $11,776,329 Head Start

The total Federal and Non-Federal amount for the Project Period is $24,240,359.

State and Local Fiscal Recovery Funds (CSLFRF) under the CFDA number of 21.027. The 
funding availability period is extended through December 31, 2021.
ii. $30,000 in additional funding to hire a Health Unit Coordinator to support staff providing 
medical care and nursing care services to homeless individuals at the Covid Isolation hotel. 
The funding for this position shall be provided under American Rescue Plan Act, Coronavirus 
State and Local Fiscal Recovery Funds (CSLFRF) under the CFDA number of 21.027. The 
funding availability period is from September 1, 2021 through December 31, 2021.
iii. $16,500 in funding to offer peer ambassadors reimbursement for time spent providing 
services to individuals experiencing homelessness. The funding for services shall be 
provided by Maricopa County Public Health Department of Epidemiology and Laboratory 
Capacity (ELC2) CFDA number 93.323. The funding availability period is from October 1, 
2021 through December 31, 2021.
10.7 The Agreement amount shall now be increased to a total not-to-exceed amount of

AMENDMENT TO IGA WITH MADISON ELEMENTARTY SCHOOL DISTRICT FOR 
SCHOOL COORDINATOR STIPEND
Approve an amendment to Intergovernmental Agreement (IGA) between Madison 
Elementary School District and Maricopa County by and through its Department of Public 
Health (MCDPH) to provide continued funding to the School Wellness Coordinator to 
implement the local wellness policy at Madison School District.
Additional funding is in the amount of $3000.00 for the budget period beginning September 
1, 2021 and ending June 15, 2022.
In addition, the IGA is amended to reflect changes as described in the amendment 1.
The IGA project is funded with AZ Health Zone SNAP-Ed dollars through the United States 
Department of Agriculture (USDA). The USDA, ADES, ADHS and Local Implementing 
Agencies work to implement policy, systems, and environmental change strategies, and shift 
the dialogue around nutrition and physical activity through marketing approaches, all with 
complementary educational outreach.

AMENDMENT TO CONTRACT WITH FRAMESHIFT GROUP
Approve an amendment to the emergency contract, PHEM 20014 with Frameshift Group, for 
the purpose of continuing COVID-19 related physician outreach services and the provision 
of expertise to schools and outpatient providers. 
The contract will be extended through April 30, 2023. The contract term began March 19, 
2020 and ends April 30, 2023. Additional funding is in the amount of $350,000 bringing the 
total not-to-exceed amount to $570,000.

It is anticipated that this amendment will be funded thru the ADHS ELC2 grant. Should the

AMENDMENT TO LEASE AGREEMENT WITH GREATER PHOENIX URBAN LEAGUE, 
INC.
Approve and authorize Chairman to execute Amendment 2 to Lease Agreement No. L-7473 
with Greater Phoenix Urban League, Inc., an Arizona non-profit corporation, (“Landlord”), for 
approximately 700 square feet of office and clinic space located at 1402 South 7th Avenue, 
Phoenix, AZ  85007.
This lease amendment will allow the Public Health Department WIC Program to continue at 
its current location. The Agreement term will be extended for one year commencing 
retroactively on October 1, 2021 and expire September 30, 2022. Unless terminated earlier 
as provided for in the Lease, there will be one automatic one (1) year renewal, to expire 
September 30, 2023, unless either party gives a 90-day written notice to the other of its intent 
not to renew. The County (“Tenant”) will pay $540.00 (Five Hundred Forty Dollars) per month 
for rent. Additionally, tenant pays a pro-rated share of utilities, trash and landscaping services 
which is anticipated to average approximately $400 per month. Tenant shall reimburse 
Landlord for the costs associated with installation of the Improvements, for a total amount 
not to exceed One Thousand Five Hundred Dollars ($1,500.00), payable within thirty days 
of receipt of invoice from Landlord. Rental tax and miscellaneous is estimated at $20 per 
month for a total not to exceed monthly average of $960 and a not to exceed amount of 
$13,020 for the 12 month budget period. The lease may be terminated for non-appropriation

COMPETITION IMPRACTICABLE CONTRACT WITH QUALTRICS, LLC
Approve a Competition Impracticable (CI) procurement award to Qualtrics, LLC. in an 
amount not to exceed $177,589.13 under Procurement Authority Code MC1-352-B. 
Qualtrics is a data survey platform that enables users to develop and disseminate surveys 
for data collection.  Qualtrics has been utilized for more than 10 years to help in data 
collection across at least five different offices within the department.  The platform allows the 
department to collect surveys from internal and external partners including but not limited to 
customer satisfaction surveys, partner evaluations, and website feedback.  We currently 
have open surveys for our Covid-19 response activities for contact tracing and symptom 
checkers.  This software platform is being used constantly by various offices in the 
department in order to protect the health and wellbeing of our staff and community. This is 
an online survey platform that does not have any installation requirements. The award period 
is August 6, 2021 and ends on August 5, 2022.

UNIFIED SCHOOL DISTRICT FOR DENTAL SEALANT SERVICES PROGRAM
Approve a non-financial Intergovernmental Agreement (IGA) between Peoria Unified School 
District and Maricopa County by and through its Department of Public Health, Office of Oral 
Health Dental Sealant Services for no cost school-based dental sealant services for 
students.  This IGA is effective beginning July 1, 2021 and remains in effect until June 30, 
2026 unless terminated by either party. The Parties have the right to extend this Agreement 
for additional one-year periods, not to exceed a total extended term of five (5) years.
The purpose of this IGA is to develop a framework of cooperation between MCDPH and 
Peoria Unified School District to provide dental sealants to eligible students for the prevention 
of tooth decay. School-based dental sealant programs are evidence-based and provide a 
safe, inexpensive model for delivering sealants to school-age children, with less learning 
time lost. Dental sealants are a proven best-practice for reducing tooth decay in the chewing 
surfaces of permanent molars. These teeth are best protected if dental sealants are applied

soon after these molars erupt (second and sixth grades). Dental sealants can protect these 
molar chewing surfaces from decay for many years.
A licensed dentist/hygienist will provide written take-home findings and a list of dental clinics 
to all (insured and uninsured) second and sixth grade students who return parental consent 
forms.  The program will then provide dental sealants to any of these same students who are 
clinically eligible and uninsured, or currently enrolled in AHCCCS/KidsCare or IHS.

funds from Arizona Department of Health Services (ADHS), in the not-to-exceed amount of 
$300,000 for the initial project period of August 2, 2021 through August 1, 2022. The project 
purpose is to expand access to services to promote healthy pregnancies and prevent 
congenital syphilis.  
This is a new award with the possibility of a 5-year term ending August 1, 2026. The award 
was non-competitive and there is no cash or in-kind match required. This is not a mandated 
service but provides a benefit to the community through the promotion of healthy 
pregnancies and the prevention of congenital syphilis. If discontinued, ongoing cash 
contributions should not be required. 
The Department of Public Health indirect rate for FY22 is 17.97%.  Full indirect costs are 
estimated at $45,698, all of which are recoverable.  Departmental indirect rates are 
reestablished at the beginning of each fiscal year and the future indirect rates will be collected 
at the corresponding rates. All program costs are allocated to the grant so there will not be 
additional burden on the department’s operating budget. 
Grant revenues are not local revenues for the purpose of the constitutional expenditure 
limitation, and therefore, expenditure of the funds is not prohibited by the budget law. 
Approval of this action does not alter the budget constraining the expenditure of local

$84,977 for a cumulative grant total of $99,977. The purpose of the project is to assess 
feasibility of implementing a “test of cure” to detect treatment failures. A ‘test of cure’ is the 
repeat testing after completion of therapy.  The project period began April 1, 2021 and ends 
July 31, 2022.    
This was a non-competitive award with no cash or in-kind match required. This is not a 
mandated portion of STD services but provides a benefit to our citizens by working to 
discover the adequacies of the CDC recommended treatment. Should the grant cease, on-

Approve Notice of Award (NOA) No. 5 NU17CE924996-03-00, from the U.S. Department of 
Health and Human Services, Centers for Disease Control (CDC) to Maricopa County by and 
through its Department of Public Health (MCDPH) to provide continued funding for work 
focused on increasing access to comprehensive and timely surveillance data on opioid 
overdose morbidity and mortality rates. The NOA provides funding in the not-to-exceed 
amount of $2,247,897 for the budget period beginning September 1, 2021 through August 
31, 2022. The term of the agreement is September 1, 2019 through August 31, 2022.   
The grant award is anticipated to be divided as follows:

Prevention - $1,752,897
Surveillance - $400,000
This is a new grant and its reoccurrence is not known. It was a non-competitive grant 
opportunity and there is no cash or in-kind match required.  Should the grant cease, on-going

to our citizens by obtaining evidenced-based data to support solutions for opioid use disorder 
prevention and treatment services for targeted underserved populations.

indirect.  Indirect cost is estimated to be $327,944, all of which is recoverable. All program 
costs are allocated to the grant so there will be no additional burden on the department’s

Chandler plans to widen Lindsay Road, from approximately 600 feet north of Ocotillo Road 
to Hunt Highway, to four lanes, with raised medians, bike lanes, center turn lane, curb, gutter, 
sidewalk, street lighting, drainage improvements and utility relocations for the Project.
The County will have a financial contribution in an amount not to exceed $1,301,229. The 
funds are budgeted in FY2027 Transportation Department (640) Transportation Capital 
Project Fund (234) Five Year Program in the Transportation System Management Project, 
sub-project TT699. Construction is planned for fiscal years 2023 and 2024.
The terms of the agreement are in effect as of the Board of Supervisors approval and shall 
remain in full force and effect until all stipulations have been satisfied, except that it may be

Appendix G (Incorporated Materials). Following the public hearing, the Board is requested 
to adopt the incorporations into the Maricopa County Air Pollution Control Regulations.
The Maricopa County Air Pollution Control Regulations contain several rules, Rules 360, 
370, 371, and Appendix G, which incorporate by reference federal regulations from Title 40 
of the Code of Federal Regulations (CFR). These federal regulations include New Source 
Performance Standards (NSPS), National Emission Standards for Hazardous Air Pollutants 
(NESHAP) and Acid Rain regulations. Every July 1, the U.S. Environmental Protection 
Agency (EPA) codifies revisions made to these federal regulations during the preceding year. 
The purpose of this rulemaking is to incorporate by reference the applicable federal 
regulations as codified on July 1, 2021, and thereby update Rules 360, 370, 371, and

Appendix G with any revisions made to the corresponding federal rules during the preceding 
year. This is necessary in order to continue to comply with the MCAQD’s delegated authority

County, except for those specific authorities retained by the EPA. After the rules are adopted 
by the Board of Supervisors, the MCAQD will request delegation authority from the EPA to 
implement and enforce the federal standards.

Supervisors'/County Manager's summary of current events - Resumen de temas de 
actualidad de los Supervisores/Administrador del Condado
Joy Rich gave a reminder to County employees to get their flu shot. The Employee 
Benefits Department have been working to get these scheduled. Health screenings are 
being offered as well. 
Supervisor Gallardo noted this is Hispanic Heritage Month. There are normally 
celebrations throughout the community, but due to the pandemic a lot of the fiestas 
have been put on hold. He said many continue to celebrate in their own way and he 
would be attending a Ball on Saturday in celebration.  He reported he was honored to 
speak to local high schools in small groups in recognition of Hispanic Heritage Month. 
Supervisor Chucri stated he had nothing to report.
Supervisor Hickman asked how many people were listening throughout the meeting. 
Debra Schaeffer, Deputy Clerk, reported there were approximately 50 people listening 
or attending virtually. He thanked his colleagues for the mutual support in the meetings 
being held.
Supervisor Gates stated there have been comments in social media that the Board of 
Supervisors needed to do more for COVID-19. He reminded the public of all that Public 
Health has done, recently making a presentation to the Board that reported the facts. 
He said the Board is focused and working with Public Health, committed to doing what 
needs to be done. 
Chairman Sellers added to Supervisor Gates’ point and stated that yesterday he had a 
conversation with Dr. Bergin, one of the people who organized getting a letter together 
and signed by 1,200 doctors and health care professionals asking the Board to do more 
to protect children in schools.  He explained the Board is working alongside Public 
Health and doing what needs to be done to control the pandemic. He reported he 
attended a moving 911 ceremony in Gilbert that was performed by Gilbert’s Public 
Safety Department. Chairman Sellers said he participated in a presentation on 
transportation at the Chandler Chamber of Commerce Public Policy Committee. He 
said it was a productive meeting and stated he was pleased with how involved the public 
is becoming in understanding the need for infrastructure.

Approve Resolution FCD 2021R02 waiving the Floodplain Regulation Administrative Fees 
for those private property owners and/or business operations located in and around Gila 
Bend, Arizona seeking such approvals to repair, replace and/or reconstruct structures 
damaged or destroyed as a result of the rains of August 13-14, 2021, for any application 
submitted prior to August 14, 2022, representing a period of one year from the date of the 
aforesaid declaration of local emergency.
This item was heard in the September 13, 2021, Executive Session. (C-69-22-133-X-00)

Director Gallardo thanked Chairman Sellers for his leadership on this agenda item. He 
said he and Director Hickman have been to Gila Bend to see the effects of the flooding.

action will assist in that effort. Flood Control will hold open house tonight to discuss 
plans moving forward.

MARICOPA COUNTY STADIUM DISTRICT POLICY S0001 PROHIBITIONS OF 
SMOKING AND TOBACCO USE ON DISTRICT PROPERTY REVISION
Approve the revision to Maricopa County Stadium District Policy Prohibitions of Smoking and 
Tobacco Use on District Property (S0001). This revision clarifies that vaping is treated the 
same as smoking.
This policy applies to District employees, volunteers, contractors, clients, partners, tenants, 
and visitors while utilizing District-owned buildings and adjacent areas. The Board of 
Supervisors is authorized to jointly adopt policies applying to the Special Districts under the 
Intergovernmental Agreement, C-06-18-393-6-00, approved on April 11, 2018. The lessee 
or tenant of District property is responsible for enforcing this policy.  This policy was originally 
approved 03/27/2019 (C-06-19-353-6-00). (C-06-19-353-6-01)
