C-22-21-056-X-01 Amendment No. 1 Page 1 of 2 AMENDMENT 1 TO AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND CHILD CRISIS OF ARIZONA I. Maricopa County (“County”) administered by its Human Services Department Early Education Division (“Department”) and Child Crisis Arizona, an Arizona non-profit corporation (“CCA”), fully executed a non-financial Agreement (“Agreement”) on or about November 4, 2020. The purpose of the Agreement is to integrate the Department’s early education services into CCA’s social and community services resource network. The Parties’ goal is to support the transition of enrolled children from CCA’s Early Head Start (“EHS”) program to the County’s Head Start program and further the children’s development. The term of the Agreement began on November 1, 2020 through June 30, 2022. The County and CCA are referred to here as the “Parties.” II. The Parties agree to amend the Agreement by this Amendment No. 1 as follows: A. Extend the Agreement term from July 1, 2022 through June 30, 2023. III. Section II above contains all the changes made by this Amendment No. 1. All other terms and conditions of the original Agreement remain the same and in full force and effect as approved and amended. IV. The Parties have authorized the undersigned to execute this Amendment No. 1 on their behalf.
