Collaborating Agency Memoranda of Understanding 
 
This Memorandum of Understanding (MOU) between Arizona Youth Partnership (AZYP) and Maricopa County by and through its 
Department of Public Health (MCDPH) (“Collaborating Agency”) is agreed as of this 1st day of July, 2022, for the purpose of 
collaborating and supporting the Health and Human Services, Administration on Children, Youth and Families-Family and Youth 
Services bureau, Maternity Group Home Program (HHS-2022-ACF-ACYF-YZ-0063) for the Starting Out Right for Homeless 
Parenting Youth Program (SOR-HPYP). The implementation of the SOR-HPYP is aimed at engaging pregnant and parenting youth, 
ages 16-22, into established Host Homes throughout Arizona, for needed housing services to improve their long-term outcomes.   
 
I. 
BACKGROUND 
 
1.1 About AZYP: AZYP is a statewide, Arizona non-profit organization whose mission is to build solid foundations for youth and 
families by partnering with Arizona communities to prevent and solve local issues such as substance abuse, youth homelessness, lack of 
educational opportunities, teen pregnancy, and challenging family dynamics. SOR-HPYP delivers an innovative approach to deliver pre- and 
post-natal health education and supportive services, including enrollment into the SOR-HPYP Host Home experience. Program 
services include 1) voluntary placement into a host home family; 2) group/individual pregnancy, childbirth, parenting, and life skills 
education; and 3) case management and ongoing supportive services including referrals for additional needed services.  
About Collaborating Agency:  MCDPH-Nurse Family Partnership empowers first-time moms to transform their lives and create 
better futures for themselves and their babies.  The expectant moms benefit by having specially educated nurses regularly visit starting 
early in pregnancy and continuing through the child’s second birthday.  New mothers develop a close relationship with a nurse who 
becomes a trusted resource they can rely on for advice on everything from safely caring for their child to taking steps to provide a 
stable, secure future for them both. The need for stability for these young mothers is vital for them to care for their babies and 
themselves.  The number one element of stability is stable housing.  The resources for stable housing for pregnancy and parenting teen 
mothers are sparce in Maricopa County and having a resource to offer our teen mothers is an essential need for them.  
 
II. COLLABORATING AGENCY ROLES & RESPONSIBILITIES 
 
2.1 AZYP/SOR-HPYP Responsibilities:  In support of the collaboration, AZYP/SOR-HPYP agrees to do the following: 
 
a. 
Provide oversite of the SOR-HPYP – Maternity Group Home Program 
b. Support SOR-HPYP efforts throughout the communities being served 
c. 
Maintain compliance with Collaborating Agency policies concerning sharing participant information 
d. Receive and provide referrals as appropriate to support those in need of stable housing 
e. 
Provide recruitment and training of qualified host home caregivers 
f. 
Follow all guidelines set forth by Collaborating Agency and report concerns immediately 
2.2 Collaborating Agency Responsibilities.  The Collaborating Agency commits to the following: 
a. 
Assign a staff person to serve as a liaison with AZYP/SOR-HPYP for this agreement 
b. Directly refer pregnant and parenting youth for voluntary involvement in SOR-HPYP and additional supportive services 
d.    Promote opportunities to become a Host Home Caregiver and provide referrals to AZYP for follow-up 
f.    Communicate issues/conflicts related to any aspect of this agreement immediately to AZYP 
  
III. MISCELLANEOUS PROVISIONS 
 
3.1 Collaborating Agency Role. The Collaborating Agency agrees that AZYP/SOR-HPYP can describe their organization in 
promotional materials and other communications as a “AZYP Collaborating Agency.” 
3.2 Media Relations.  Collaborating Agency agrees to provide AZYP with a draft of media releases regarding Starting Out Right 
Homeless Parenting Youth Program for approval prior to submission to media outlet. 
3.3 Relationship of the Parties.  Nothing in this agreement is intended to or shall be construed to constitute or establish an agency, 
joint venture, partnership, or fiduciary relationship between the parties and neither party shall have the right or authority to act for or 
on behalf of the other party. 
3.4 Compensation. No compensation will occur with this agreement.   
3.5 Governing Law.  The validity, interpretation, construction, and performance of this agreement shall be governed by the laws of 
Arizona without regard to its conflicts of law principles. 
3.6 Term. This agreement is in effect until June 30, 2023. A new MOU may be issued by AZYP each year upon the notification of 
funding continuation from ADHS. This MOU may be terminated by either party upon 60 days prior written notice.  
3.7 Limitation of Liability. Neither party shall be liable to the other for any indirect, incidental, reliance, special punitive or 
consequential damages, including loss of profits, revenue, prospective business, data, or use, incurred by either party, whether in an 
action in contract or tort, even if the other party or any other person has been advised of the possibility of such damages.
