amended 
Dear Mr. Smith, 
This letter summarizes administrative changes to the Revocable License Agreement, as referenced 
above, located at 6535 East Osborn Road, Scottsdale (Facility).   Due to the increasing needs of 
the City, Licensee’s Use Area will be relocated from Building 1 to Building 7 within the City of 
Scottsdale’s Neighborhood Paiute Center.  Exhibit “A” to the Agreement shall be deleted in its 
entirety and replaced with Exhibit “A”, enclosed hereto and incorporated by this reference.  Each 
Party to the Agreement approves the new Use Area as amended in “Exhibit A” to the Agreement 
and is to be signed by each Party’s administrative authority.  
The new Use Area will have a reduction in square footage, from 143 to 130 square feet of licensed 
space.  As a result of the Use Area square footage reduction the Use Fee will also be reduced 
effective July 1, 2022 through October 31, 2022.

Use Area:  Portion of the Facility inside the Scottsdale Family Resource Center - Building 7 for 
the exclusive use of 130 square feet of enclosed office space. 
Times of Use: The Licensee may use the Use Area from 7:00 a.m. to 7:00 p.m. on Mondays 
through Thursdays and from 7:00 a.m. to 6:00. p.m. on Fridays. The City reserves the right to use 
the Use Area at times other than at the Times of Use by the Licensee.
