AMENDMENT TO
NON-PROCUREMENT CONTRACT
FOR FEDERALLY QUALIFIED HEALTH CENTERS

ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS

The above-named contract is hereby amended as specified below:

A. This amendment to the non-procurement Contract for Federally Qualified Health
Centers to provide grant assistance program related to services for the COVID-
19 Pandemic will increase funding by $27,000,000 ($26,550,000 for grant
assistant programs + 450,000 for AACHC operational expenses). The total
amount not-to-exceed is $52,000,000.

B. This Amendment modifies page one (1) of the original agreement in the following
manner:
Contract not-to-exceed from $25,000,000 to $52,000,000.
Contract termination date is from December 31,2024 to June 30, 2026.

Cc. Amend Section 3.1 - delete section in entirety and replace with the following:
The Contractor shall be responsible for awarding the Federally Qualified
Health Centers who were approved but previously unawarded through the
first awarding cycle. Funded proposals shall provide the same Program
Scope and Requirements mentioned in Attachment A.

D. Delete section 3.2.

G. Amend Section 10.2 -- delete verbiage “wire transfer’ and replace it with
disburse.

H. Amend Section 5.0 Term-- The Contract end date is extended from December 31,
2024 to June 30, 2026

|. Amend Section 10.4 — delete $787,520 and replace with $1,237,520.

J. Amend Section 10.5.2 — delete section and replace with “10.5.2 -- $150,000 on
July 1, 2022”

K. Amendment Section 10.5.3 - delete section and replace with “$400,000 on
January 1, 2023;

L. Add Section 10.5.4 to include -- $387,520 on January 1, 2024;

All other terms and conditions of the original contract shall remain in full force and
effect.
