It is mutually agreed that the Intergovernmental Agreement referenced is amended as follows:
1.
Pursuant to Terms and Conditions, Provision Four (4), Contract Administration and Operation, Section 4.1., Term,
the termination date is extended to October 31, 2022.
2.
Pursuant to Terms and Conditions, Provision Six (6), Contract Changes, Section 6.1, Amendments, Purchase
Orders and Change Orders, the Agreement is hereby amended as follows:
2.1. The Scope of Work (SOW) is hereby amended and replaced with the new SOW of this Amendment Two (2),

1.
Background

The Arizona Department of Health Services (ADHS), Environmental Public Health Tracking (EPHT) Program, hopes
to expand capacity of the existing public portal (www.azdhs.gov/epht) by working with local public health partners.
The portal is a tool to help Arizonans learn about environmental hazards in the state that could impact their health.
The Program has gathered data from multiple sources for Arizonans to view both environmental and health outcome
data in one easily accessible place. Data on hospitalizations, emergency department visits, air quality, blood lead,
carbon monoxide poisoning, extreme heat days, flood vulnerability, heat-related illness, myocardial infarction,
mortality, morbidity, natality, and demographics can be viewed as maps, graphs, rates, and tables. Data and
corresponding metadata can also be downloaded. Additional layers such as schools, hospitals, and fire potential
can also be overlaid. The Program hopes to maintain required nationally consistent data and measures (NCDM)
and add optional NCDM. The Program will also like to advance analysis and increase use of the data for public
health action by communication strategies, teaching, and partnering for advanced analyses on the data. The
Program hopes to work with local partners for small area analyses that could aid informing public health
interventions by the agency and partners. Funding for this project is provided through a cooperative agreement with
the Centers for Disease Control and Prevention (CDC) under funding opportunity announcement CDC-RFA-EH-
17-1702, “Enhancing Innovation and Capabilities of the Environmental Public Health Tracking Network.” The
Arizona Department of Health Services was awarded funding as a grantee under award #1 NUE1EH001339-01-00
to perform the project between 8/1/17 – 7/31/22.

2.
Objective

Maricopa County Department of Public Health (MCDPH) Office of Epidemiology has unique experience in
environmental analysis that would complement the Arizona Environmental Public Health Tracking Team. As a
jurisdiction which covers 60 percent of Arizona's population, they are in a unique position to reach a large portion
of the state's vulnerable populations to environmental hazards. MCDPH's epidemiological surveillance expertise
includes Dr. Vjollca Berisha Senior Epidemiologist and Dr. Jessica White, Public Health Scientist who have
experience publishing heat morbidity and mortality surveillance findings and overlaying environmental hazard
information. MCDPH is developing a strategic plan on environmental hazards. Tracking would complement their
existing efforts. MCDPH will become a sub-grantee on this project and assist in collection, data management, and
analysis of required and optional EPHT Nationally Consistent Data and Measures (NCDM) for dissemination to
local stakeholders, such as vulnerable populations at hazardous waste sites in Maricopa and areas with air and
water quality concerns. A pilot project for testing the feasibility of formatting required and optional NCDM indicators
will be implemented with the assistance of MCDPH. This pilot will be used as a means of identifying if the strategies
would be effective for applying statewide. Under CDC-RFA-EH-17-1702, grantees are required to complete
advanced activities outlined in the funding opportunity announcement at a minimum of two (2) per budget year.
MCPDH will assist ADHS on completing these advanced activities.

3.
Scope of Service

The project will function on an annual budget period concurrent with the grant budget period. The geographic scope
of the project is county scale. The total length of the project period is for five (5) budget years. The first year budget
will be a condensed year starting upon signature of the intergovernmental agreement (IGA) through July 31, 2018
(Year 1). In the following years the project’s budget year will be (Year 2) August 1, 2018 – July 31, 2019, (Year 3)
August 1, 2019 – July 31, 2020, (Year 4) August 1, 2020 – July 31, 2021, (Year 5) August 1, 2021 – July 31, 2022.
Continuation of the project after Year 1 is dependent on cooperative agreement funding availability from CDC-RFA-
EH-17-1702. A revised scope of work based on funding availability will be agreed upon with sub-grantees before
the start of each budget year on an annual basis prior to the start of the next budget year.

4.
Tasks

4.1. Year 1 (Upon Signature – July 31, 2018)

ADHS and the sub-grantee shall:

4.1.1.
ADHS EPHT Program and MCDPH Office of Epidemiology related staff shall meet in person or
through teleconference quarterly regarding project deliverables; and comply with all state and federal
regulations.

4.1.2.
ADHS Responsibilities:

4.1.2.1.
Meet project deadlines, ADHS will regularly keep up with communications with the sub-
grantee staff in order to direct priorities; and write NCDM most suited for a public health
expert within ADHS.

Maricopa County shall:

4.1.2.2.
Continue activities from Year One (1) and updating NCDM for state and national tracking
network;

4.1.2.3.
Maintain advanced activities completed in Year One (1) defined in CDC-RFA-EH-17-1702
through Year Five (5);

4.1.2.4.
Assist ADHS in completing two (2) additional Advanced activities per year until maintaining
all ten (10) defined in the notice of funding opportunity CDC-RFA-EH-17-1702. The ten
(10) activities are defined below:

4.1.2.4.1. Address gaps or limitations in available data by collaborating with state and
local programs to optimize data collection and ensure the inclusion of data
elements relevant to environmental public health tracking; integrating novel or
non-traditional data determined to be relevant and of sufficient quality (e.g.,
electronic health record data); conducting analyses and activities that either fills
the identified data gap or extends the applicability of the available data; or
expanding the availability and utility of sub-county data and measures,
4.1.2.4.2. Conduct routine analyses using Tracking data for the discovery of spatial,
temporal, and demographic patterns of diseases, exposures, or environmental
hazards; the timely identification of unusual events signaling the need for further
investigation; or the non-research purposes of analyzing or linking health,
exposure, and hazard data to help provide new insights or solutions to
environmental health issues,
4.1.2.4.3. Apply and interpret Tracking data to inform the development and evaluation of
public health actions,
4.1.2.4.4. Establish repeatable, reliable, and readily accessible electronic access to data
sources, including tools and processes to increase the availability and
accessibility of timely and locally relevant data (e.g., sub-county and real time
data) and any needs discovered as the result of the gap analysis,
4.1.2.4.5. Increase data management efficiency by developing state/local standard
operating procedures and increasing the use of routine, reusable, and
electronic processes and scripts for obtaining, validating, and disseminating
data, including use of application programming interfaces,
4.1.2.4.6. In the communications plan, maintain at least one collaborative communication
activity annually, in addition to the program-wide activity, with one or more

4.1.2.4.7. Conduct a needs assessment with target audience(s) to inform development of
at least two custom communication products (e.g., messages, tools) to
communicate Tracking data and information. Products may include, but are not
limited to fact sheets, environmental health snapshots, infographics, newsletter
articles, and social media posts,
4.1.2.4.8. Establish and maintain relationship with local media (i.e., newspapers,
magazines, TV) to promote Tracking through earned media,
4.1.2.4.9. Co-present/Co-sponsor a national or regional meeting/conference or webinar
with other grantees or partners, and
4.1.2.4.10. Develop and implement program-specific training opportunities, through Web-
based or in-person training modules or other product (e.g., video tutorial) and
internships/fellowships/mentorships for students in environmental public health
tracking.
4.1.3.
Sub-grantee Responsibilities (Upon Signature – July 31, 2018)

4.1.3.1.
MCDPH Office of Epidemiology Technical Assistance shall:

4.1.3.1.1. Work under guidance and direction of ADHS staff;

4.1.3.1.2. Identify staff to provide expertise on a technical advisory workgroup for Tracking
which consists of identifying stakeholders and events for applying Tracking data
for public health action and participating in workgroup meetings;

4.1.3.1.3. Participate in developing and implementing presentations and trainings using
Tracking data with public health staff, vulnerable populations, and other
relevant stakeholders;

4.1.3.1.4. Provide recommendations based on data elements and feasibility of collecting
data;

4.1.3.1.5. Dedicate a percentage of existing management staff for oversight of
administrative duties for the senior epidemiologist;

4.1.3.2.
Epidemiology and Statistics Capacity for Environmental Public Health Tracking
(EPHT) Nationally Consistent Data and Measures (NCDM) Analyses shall:

4.1.3.2.1. Enhance epidemiology and statistics capacity for the EPHT Program by hiring
and maintaining a full-time employee (FTE) Senior Epidemiologist with at least
a Master’s level degree (MPH) or equivalent with two (2) years of experience
or a PhD level whom can complete the required NCDM as well as analyze
NCDM in order to identify emerging public health concerns. This position will
also have biostatistics training to complement epidemiological analyses.
MCDPH can also use a temporary staffing contractor to meet project needs
during gap periods;

4.1.3.2.2. Ensure that in addition to participating in tasks under 4.1.3.1, the Senior
Epidemiologist duties will encompass items 4.1.3.2.3 - 4.1.3.2.6.4;

4.1.3.2.3. Ensure that the Senior Epidemiologist shall participate in CDC Environmental
Public Health Tracking Content Working Groups and sub-groups related to
collecting and analyzing existing and optional NCDM;

4.1.3.2.4. Create metadata reports for each NCDM yearly dataset produced using CDC
EPHT Metadata Standards;

4.1.3.2.5. Utilize programming languages such as SAS and SQL to perform data
validation techniques to ensure data is clean and accurate;

4.1.3.2.6. Provide surveillance of environmental and health outcome data required by the
Tracking Network that includes:

4.1.3.2.6.1. Analyses of NCDM using methods identified using CDC EPHT
How-To-Guides and Indicator Templates;

4.1.3.2.6.2. Applying how-to-guides and indicator templates to Arizona data
for acute myocardial infarction, air quality, asthma, birth defects,
cancer, carbon dioxide poisoning, childhood lead poisoning,
drinking water, heat, and reproductive outcomes;

4.1.3.2.6.3. Formatting and submitting data to the National EPHT Network and
Arizona Tracking Portal; and

4.1.3.2.6.4. Developing how-to-guides and indicator templates for new NCDM
with other EPHT grantees.

4.1.3.3.
Analyze and Apply Data to Generate Evidence-based Activities

4.1.3.3.1. The senior epidemiologist shall conduct the following activities in 4.1.3.3;

4.1.3.3.2. Conduct routine descriptive analysis of Tracking data to evaluate quality and
completeness of data; to examine state and local environmental public health
issues; or to generate information for other state and local programs, state and
local decision makers, and the public;

4.1.3.3.3. Use Tracking data to identify and prioritize the jurisdictions’ environmental
public health concerns and priorities, and need for action to help direct Tracking
program activities including but not limited to those listed in the Funding
Opportunity Announcement (CDC-RFA-EH-17-1702);

4.1.3.3.4. Advanced Activity: Conduct routine analyses using Tracking data for the
discovery of spatial, temporal, and demographic patterns of disease,
exposures, or environmental hazards; the timely identification of unusual events
signaling the need for further investigation; or the non-research purpose of
analyzing or linking health, exposure, and hazard data to help provide new
insights or solutions to environmental health issues;

4.1.3.4.
Identify and Integrate Public Health and Environmental Data

4.1.3.4.1. The senior epidemiologist shall conduct the following activities in 4.1.3.4;

4.1.3.4.2. Identify and evaluate the strengths and limitations of health, exposure,
environmental, and other data for the Network, in addition to work conducted
as part of the CDC EPHT Content Workgroup;

4.1.3.4.3. Integrate data into grantee’s Tracking Network and ensure the availability and
accessibility of data as appropriate for program staff and internal and external
partners;

4.1.3.4.4. Maintain existing and newly adopted Nationally Consistent Data and Measures
requirements including maintenance of NCDM data within the grantee tracking
network, submission of NCDM data to CDC, and dissemination of required
NCDM measures via grantee’s Tracking Network;

4.1.3.4.5. Disseminate additional indicators and measures to address grantee’s priorities,
optional NCMS or other agreed-upon standards;

4.1.3.4.6. Collaborate with CDC, data stewards, data users, and other tracking grantees
to improve existing NCDM’s and, adopt new and optional NCDM’s as developed
by the CDC EPHT Content Working Group including traveling to a national
conference such as a grantee meeting to showcase projects and within state
conferences and meetings; and

4.1.3.4.7. Advanced Activity: Address gaps or limitations in available data by collaborating
with state and local programs to optimize data collection and ensure the
inclusion of data elements relevant to environmental public health tracking;
integrating novel or non-traditional data determined to be relevant and of
sufficient quality; conducting analyses and activities that either fills the identified
data gap or extends the applicability of the available data; or expanding the
availability and utility of sub-county data and measures;

4.2. Year 2 -5 (August 1, 2018 – July 31, 2022)

4.2.1. A revised agreed upon scope of work will be implemented prior to beginning Year Two (2). Continuation
of the project is dependent upon funding availability. The following bullets represent a summary of
strategies that will be further addressed in more detail in Years two through five (2-5),

4.2.1.1. Continue updating NCDM for state and national tracking network;

4.2.1.2. Provide technical assistance for descriptive epidemiological and biostatistics surveillance
reports on required and optional NCDM;

4.2.1.3. Actively participate in relevant local, state, and national EPHT workgroups.

4.2.2. Arizona Heat-Related Illness Dashboard: Lead the completion of the pilot project to develop an Arizona
heat-related illness dashboard utilizing 2022 syndromic surveillance data from emergency departments
visits for internal stakeholders and local health departments utilizing Tableau visualization tools,

4.2.3. LEAD Project: Completion of the pilot project to bring one NCDM dataset (asthma emergency department
visits for years 2010-2019) to be displayed by census tract level on the Arizona Data Explorer/Public
Health Data Portal, and

4.2.4. Content Workgroup (CWG): Completion of EPHT CWG projects for current CWG cycle for ESSENCE
Heat-related Illness and Asthma Sub-team. Projects include (1) final deliverable summarizing workgroup
progress and recommendations and (2) an abstract for the 2022 Syndromic Surveillance Symposium to
all be completed in July 2022.

5.
Deliverables and Delivery Schedule
The Contractor shall:
5.1. Provide and submit to the ADHS an invoice of the following deliverables in accordance with the price sheet of the
fixed price line items;
5.2. Complete the contractor expenditure report (CER) of all deliverables and reporting requirements thirty (30) days
after completion of the price sheet item;
5.3. Participate in partner meetings as scheduled by Program Manager;
5.4. Complete and submit quarterly surveillance reports for EPHT activities;
5.5. Submit quarterly activity evaluation reports which describe workgroup participation and contribution towards
meeting grant milestones using evaluation metrics to the Program Manager;
5.6. Provide quarterly surveillance and evaluation report for first quarter will be a shortened timeframe due to contract
implementation occurring during middle of grant budget year;
5.7. Provide communication of EPHT products on price sheet for in-state and out-of-state events can be invoiced after
completion of accepted presentation and local and national conference or meeting agreed to by the Program
Manager; and
5.8. Indirect costs can be invoiced yearly after completion of all deliverables for each budget year.
