In accordance with the contract terms and conditions, your company was selected to be the most advantageous to the State, taking into consideration the evaluation factors set forth in the Request for Grant Application. In accordance with the contract terms and conditions, Maricopa County (Maricopa County School Superintendent’s Office) has 10 days from the date of this letter to submit a Certificate of Insurance. The certificate must indicate compliance with the insurance requirements set forth in the Terms and Conditions, Page 13. Please include the contract number on the Certificate of Insurance.
