MARICOPA COUNTY, ARIZONA 
 
WHEREAS, Maricopa County (“County”) and Health Choice AZ Inc., (“Contractor”) have entered into an Agreement for 
the purpose of establishing collaborative protocol executed March 23rd , 2022 (“Agreement”). 
 
WHEREAS, County and Contractor have agreed to renew this Agreement;  
 
NOW, THEREFORE, the parties hereby agree to the following provisions: 
 
 
1. 
The Agreement shall be renewed retroactively for a term from May 17th, 2020 and ending May 16th, 2022.  
2. 
All references to “Steward Health Choice America” in this Agreement shall be replaced with the new name 
of “Health Choice AZ Inc.” 
 
 
ALL OTHER TERMS AND CONDITION REMAIN UNCHANGED. If there is any conflict between the terms of 
the Agreement and this Amendment, in all such events the terms of this Amendment shall control. 
 
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511. 
 
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both
