Amendment No. One (1) to the 2021-2022
Independent Contractor Agreement
Contract No. 07012021--22
The Independent Contract Agreement dated July 1, 2021, between Arizona Community Action
Association (dba Wildfire) and Maricopa County, administered by the Human Services
Department, Housing and Community Development Division (HCD)
(hereinafter
“Contractor”) to conduct application intake and eligibility determination for utility bill assistance
and deposits is hereby amended as follows:
Purpose of the Amendment:
1. To increase Utility Repair Replacement Deposit (URRD) funds by the amount of
$50,000.
Amendment to:
Section I. Services and Programs – 1.2 Fund Sources; Attachment 1:
Utility Repair Replacement Deposit (URRD): To increase the Direct Service amount by $45,455
and increase the Program Delivery amount by $4,545. The new additional allocation for URRD
will be $50,000 (Direct Service + Program Delivery). Total new contract amount will be
$342,690 (New allocation of $50,000 + Initial allocation of $292,690).
Whole Agreement:
Unless otherwise noted herein, all other provisions of the original Agreement will remain in place
for the duration of the original Agreement.
