AMENDMENT NO. 2 TO THE  
CONTRACT 
BETWEEN 
MARICOPA COUNTY  
ADMINISTERED BY ITS HUMAN SERVICES  
DEPARTMENT  
AND 
SOLARI, INC. 
 
I. 
Maricopa County (“County”) by and through the Maricopa County Human Services 
Department and Solari Inc. (“Contractor”) entered into a Contract on or about August 17, 
2021, to provide additional guidance to persons who are applying or have applied for 
County-administered rental and utility assistance. The County provided the Subrecipient 
with $25,000 in American Rescue Plan Act Coronavirus State and Local Fiscal Recovery 
Funds under CFDA number 21.027 for these services. The County and the Contractor 
may be referred to individually as “Party” or collectively as “Parties.”  
 
The Parties executed Amendment No. 1 on or about October 4, 2021. The purpose of 
Amendment was for the County to provide an additional $25,000 in American Rescue Plan 
Act Coronavirus State and Local Fiscal Recovery Funds under CFDA number 21.027 for 
the Contractor to conduct additional customer service calls to landlords on behalf of 
tenants in the process of receiving rental and/or utility assistance to prevent evictions. 
 
II. 
The Parties now agree to amend the Agreement as follows: 
 
A. 
Revise Section 2.0 (PURPOSE), by removing in its entirety and replacing with the 
following: 
The purpose of the Agreement is to assist the County in providing services to those 
who have been disproportionately impacted by the pandemic through an 
expansion of 211 services including but not limited to transportation services, 
housing crisis hotline, and assistance to those facing eviction.  
 
B. 
Revise Section 3.0 (CONTRACTOR RESPONSIBILITIES), by removing and 
replacing subsection 3.1 and 3.2 of the Agreement as follows: 
3.1 
The Contractor shall complete the following activities: 
3.1.1 Emergency Rental Assistance Program Support 
3.1.1.1 The Contractor shall contact County-identified persons 
(which may include landlords or other parties) regarding 
eviction prevention services and programs, which may 
include but is not limited to providing guidance for properly 
completing the rental and/or utility assistance applications. 
3.1.2 Transportation Navigation and Direct Assistance 
3.1.2.1 Provide transportation services and ride fares for Maricopa 
County residents to meet a person’s social determinants of 
health needs. 
3.1.3 Housing Crisis Hotline 
3.1.3.1 Operate and market an inbound phone number to provide 
navigation support for any housing crisis.

3.1.3.2 Assess housing needs and assist callers in getting 
connected to bridge housing and homelessness services. 
3.1.4 The Contractor shall work with the County in the development and 
implementation of the activities. 
3.1.5 The Contractor shall provide the services and follow the project 
scope as identified in revised Attachment-A (211 Arizona 
Expanded Services for Maricopa County), hereby incorporated into 
the Agreement.  
3.2 
The Contractor shall track and provide the County a monthly report as 
detailed in Attachment A Deliverable Matrix. 
3.2.1  For the Emergency Rental Assistance Program Support activity, the 
following information shall also be provided: 
3.2.1.1 Monthly report listing of contact attempts (emails and phone 
calls to applicants). This report shall include the applicant’s 
name, identification number, type of contact (email or 
phone), and date contacted. Additionally, this report shall 
include key information based upon agreed categories, 
including reason the applicant did not complete the 
application and final applicant disposition  
3.3 
The Contractor shall provide the County with all deliverables according to 
the agreed upon timetables noted in Attachment-A. 
 
C. 
Revise Section 4.0 (COUNTY RESPONSIBILITIES) of the Agreement by adding 
the following to subsection 4.1:

regarding 
the 
development 
and 
implementation of activities. 
 
D. 
Revise Section 5.0 (TERM) of the Agreement by removing in its entirety and 
replacing with the following: 
The Agreement shall be effective on the date listed on page one of this Agreement, 
and will expire on December 31, 2022. 
 
E. 
Revise Section 10.0 (FUNDING, INVOICING, AND PAYMENT), by removing in its 
entirety and replacing with the following:  
10.1 
The County shall provide funding under Catalogue of Federal Domestic 
Assistance (CDFA) 21.027 American Rescue Plan Act State and Local 
Fiscal Recovery Funds provided to the County through the U.S. 
Department of Treasury.  
10.2 
The Agreement total funding amount is $2,987,030, which is separated by 
activity as follows: 
10.2.1 $1,285,970 for Emergency Rental Assistance Navigation Support; 
10.2.2 $1,267,566 for the Housing Crisis Hotline; and 
10.2.3 $383,494 for the Transportation Navigation and Assistance 
Services.  
10.3 
The Contractor shall submit invoices for the actual costs incurred for these 
activities monthly supported by back-up documentation.  The expenditures 
and invoice shall be separated by activity as listed in 3.1.  
10.4 
The Contractor shall submit to the County a Request for Reimbursement 
of all expenditures within the same fiscal year in which the expenditures

are incurred. The fiscal year runs July 1st through June 30th and all 
Requests for Reimbursement shall be submitted no later than July 30th 
for the preceding fiscal year.  
10.5 
The Contractor must submit the data by activity as detailed in 3.2 to 
receive reimbursement as detailed on the monthly invoice. 
10.6 
The Contractor shall submit invoices to hsdfinance@maricopa.gov. 
10.7 
The County shall reimburse the Contractor on a Net 0 payment standard. 
10.8 
Disallowed costs as detailed in the Uniform Administrative Requirements 
as listed in Section 35 shall not be submitted for reimbursement by the 
Contractor and may not be reimbursed with County funds. 
 
III. 
This Agreement is subject to A.R.S. §38-511. The foregoing paragraphs, contain all the 
changes made by this Amendment. All other terms and conditions of the original 
Agreement and Amendment No. 1 remain the same and in full force and effect as 
approved and amended.
