AMENDMENT TO INTERGOVERNMENTAL AGREEMENT (IGA) WITH
CITY OF PHOENIX FOR MASS IMMUNIZATION SERVICES AND ADMINISTRATION 
I.
The above-named Intergovernmental Agreement (IGA) is hereby amended as specified 
below:
A.
The IGA period is extended thru March 31, 2023.
B.
Section IV Compensation, 1. Compensation, Letter F on Page 17 of the Original IGA has 
been replaced and revised with the revised Section IV Compensation, 1. Compensation, 
Letter F on Page 2 of this Amendment.
C.
The total approved funding amount for the City of Phoenix will remain unchanged.
II.
All other terms of the IGA remain unchanged.
