The above-named contract is hereby amended as specified below:
A. Exhibit G – Special Projects to the Master Agreement originally dated July 28,
2020, and modified in Amendment No. 2 dated January 12, 2022, is amended as
follows:

Scope of Work
In addition to the scope of work outlined in Amendment 2, Exhibit G, the following
is added:

a.
Provide special project surge capacity support for MCDPH staff to conduct
targeted COVID-19 case investigations for populations experiencing
especially high case counts. Populations to be identified by MCDPH.
b.
Assistance includes training/retraining of staff to conduct case
investigations, collecting data and entering data in MEDSIS, auditing data
collected for ongoing improvement and control, and providing daily
communications to MCDPH. All staff engaged in this activity will sign a
confidentiality agreement and take appropriate confidentiality training.
c.
ASU will not collect any data beyond what is included on the original
MCDPH form. If there is a request for ASU to collect data beyond what is
included in the original MCDPH form, then an amendment to this Exhibit will
be processed and mutually agreed upon between ASU and MCDPH.
MCDPH shall have full access to and the right to examine, copy, and make
use of any and all such data.
d. ASU will be using the MCDPH supplied Qualtrics questionnaire for
collection of data. ASU will use reasonable efforts to collect complete data
from each case interviewed including disease specific observations (DSO).
