AMENDMENT NO. 2 TO THE 
INTERGOVERNMENTAL AGREEMENT 
BETWEEN 
MARICOPA COUNTY 
ADMINISTERED BY ITS 
HUMAN SERVICES DEPARTMENT 
AND 
THE ARIZONA FIRE AND MEDICAL AUTHORITY 
 
I. 
Maricopa County (“County”), administered by its Human Services Department, entered 
into a financial Intergovernmental Agreement (“Agreement”) with the Arizona Fire and 
Medical Authority (“Subrecipient”) on or about June 23, 2021. The purpose of the 
Agreement is for the Subrecipient to install a back-up electrical generator at the Tonopah 
Fire Station. The County has provided the Subrecipient with $107,374 in U.S. Department 
of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) 
funds. The Agreement term began on July 1, 2021 through June 30, 2023. The County 
and the Subrecipient collectively are referred to as the “Parties.” 
 
The Parties fully executed Amendment No. 1 on or about September 15, 2021. Among 
other things, it provided $107,374 to the Subrecipient to install a back-up electrical 
generator at the Wittman Fire Station. The funding was provided by U.S. Department of 
Housing and Urban Development (HUD), CFDA 14.218, Community Development Block 
Grant (CDBG). 
 
II. 
The Parties agree to amend the Agreement by this Amendment No. 2 as follows: 
 
A. 
Revise Section 3 (Work Statement), Paragraph 2.0 (Scope of Work) delete in its 
entirety and replace with the following: 
2.1 
Project Description: 
2.1.1 The Subrecipient shall purchase and install a back-up generator for 
the Tonopah, Arizona fire station. The new 80K generator will 
provide electricity during a power failure. The generator will provide 
electricity to operate the building safety equipment, allow the fire 
station to respond to emergencies during power outages, including 
but not limited to radio communications, mobile computer terminals, 
operating bay doors, building safety lighting, well water system, and 
HVAC units. 
2.1.2 The Subrecipient shall purchase and replace the well water system 
and parts. The well is the fire station’s only source of water which is 
used in the fire station for all water-related needs, as well as the 
water needed for the tanks on emergency response vehicles. The 
improvements include a storage tank, a pressure tank, and a 
pressure pump. 
2.1.3 The Subrecipient shall install security perimeter fencing around the 
fire station and its resources which include the fire station 
apparatus, a back-up generator, buried electrical cables, air 
condition units, and a well water system to help secure the 
premises. 
2.2 
Project Purpose: The Authority is purchasing and installing the new 80K 
back-up generator to ensure the safety and ability to respond to 
emergencies during a power failure; replace the old and dilapidated well

water system and parts to create a more efficient and operable water 
system for the fire station; and install security fencing around the perimeter 
to help secure the fire station and valuable resources which are vital to the 
service provided to the residents of Tonopah community. 
2.3 
Project Beneficiaries: The project directly supports the public health and 
safety of 1,105 Tonopah area residents living in low-to-moderate income 
households. The fire station supports more than 300 square miles, 
including Tonopah as well as Interstate 10. 
2.4 
Project Staff: The Subrecipient shall maintain staff qualified to perform the 
duties of the project. The Subrecipient shall immediately notify the County 
regarding any changes in staff committed to the CDBG project. The County 
reserves the right to review the qualifications of new staff committed to the 
CDBG project after the execution of this Agreement. 
2.5 
Subcontractors: The Subrecipient shall select Subcontractors in 
accordance with the Administrative Requirements of this Agreement. The 
Subrecipient shall contract with responsible and qualified Subcontractors 
to perform the duties of the project. The Subrecipient shall verify the 
qualifications of each Subcontractor through license verification, 
references, and SAM.gov. 
 
B.

to the funding reimbursement amounts per fiscal year, the Parties 
shall process an Administrative Change Order identified in Section 
2 (Special Provisions), Sub-subparagraph 13.3.18 (Administrative 
Change Orders). 
 
D. 
Funding for Amendment No. 2 is provided by U.S. Department of Housing and 
Urban Development (HUD) Community Development Block Grant (CDBG) funds. 
 
III. 
The Agreement is amended to incorporate the changes contained in this Amendment No. 
2. All other terms and conditions of the Agreement and previously approved Amendments 
shall remain unchanged and in full force as approved and amended. 
 
IV. 
The Parties have authorized the undersigned to execute this Amendment No. 2 on their 
behalf, and it shall be effective upon approval and signature by both Parties.
