Term and Termination to the Master Intergovernmental Agreement originally dated July 28, 2020 and modified 
in Amendment No. 1 dated Feb 17, 2021 is amended as follows:
3.1 is deleted in its entirety and replaced with the following:
The term of this Agreement will end on June 30, 2022 and will terminate as provided herein. The 
parties may modify or extend this Agreement at any time by mutual written consent, including in 
any SOW.
Exhibit D – Case Investigation to the Master Agreement originally dated July 28, 2020, and modified in 
Amendment No. 1 dated Feb 17, 2021 is amended as follows:

Period of Performance is extended to June 30, 2022.
3.
Scope of Work is amended as follows:
ASU will provide surge capacity for MCDPH staff for COVID-19 Case Investigations and/or other
reportable infectious diseases as needed. Assistance includes ongoing modifications to training
materials in line with changing public health guidance, training/re-training of staff to conduct case
investigations, collecting data and entering data in MEDSIS, auditing data collected for ongoing
quality improvement and control, and providing daily communications to MCDPH. Any and all
students involved will sign a confidentiality agreement and take appropriate confidentiality 
training.
ASU will not collect any data beyond what is included on the original MCDPH form. If there is a
request for ASU to collect data beyond what is included in the original MCDPH form, then an
amendment to this Exhibit will be processed and mutually agreed upon between ASU and MCDPH.
MCDPH shall have full access to and the right to examine, copy, and make use of any and all such
data. 
ASU will be using the MCDPH supplied Qualtrics questionnaire for collection of data.
ASU will use reasonable efforts to collect complete data from each case interviewed including 
disease specific observations (DSO).  To achieve an optimal response rate during COVID-19 case 
investigations, the following contact methodology will be attempted:

The student will attempt a maximum of one (1) documented phone call and an additional 
phone call or text message over a 24-hour time period.

•
After two (2) unsuccessful attempts at contact, the case will be closed and entered as lost 
to follow-up in MEDSIS.
•
If for any reason the contact information is incorrect or the phone number is non-working, 
the case should be submitted to ADHS after one (1) attempt and a note of the MEDSIS ID 
should be sent to MCDPH in the end-of-shift interview.
In order to support the necessary level of information security standards related to this project’s 
data collection, processing, and storage requirements, the ASU team will:
•
Unmanaged endpoint systems, used for the volunteer program, will not be subject to 
enforceable security controls at the host environment level, although volunteers are 
advised to follow university best practices for personal equipment.
•
Volunteers must comply with mandatory HIPAA training and pass the examination.
•
SharePoint data collaboration systems used to support this effort must:
a.
Limit and ensure data collected from volunteers does not identify patient health status 
via deidentification, minimization of patient identifier storage, and obfuscation or 
exclusion of intermediary purposes.
b.
Implement appropriate role-based access controls and permissions must be applied to 
limit access to only the relevant personnel.
c.
Implement multi-factor authentication (MFA) with ASU credentials.
d.
Archive securely or remove all data after the life of the project.

The goal of this activity is to ensure continued, real-time capacity for timely responses to emerging public 
health priorities. This is accomplished through four project areas of work: 

Regular reviews of newly published “high-impact” COVID-19 findings in the scientific literature. a. 
“High-impact” is to entail both top-tier scientific journals (JAMA, New England Journal of Medicine, 
The Lancet, Health Affairs) and studies that have been widely reported by the popular press. These 
reviews can help to ensure MCDPH is aware of emerging findings. Reviews will largely be focused 
on the methodological detail of these studies—including strengths and weaknesses. Written review 
documents will be prepared and sent approximately weekly to the MCDPH Intelligence section 
leadership and other interested parties as mutually agreeable.

Both written and narrative summaries will be regularly provided for departmental personnel 
leading Maricopa County’s COVID-19 disease control and investigation.

Continued availability to respond to requests for evidence/literature reviews on emerging COVID-
19 topics.

Statistical analyses of mutually agreeable topics related to the COVID-19 burden of disease in 
Maricopa County.

Additional support as mutually agreeable to review and provide written or narrative summaries of 
the COVID-19 scientific literature on behalf of other departmental stakeholders.
