AMENDMENT TO INTERGOVERNMENTAL AGREEMENTS (IGA) WITH CITY OF PHOENIX FOR MASS 
IMMUNIZATION SERVICES AND ADMINISTRATION 
The above-named contract is hereby amended as specified below:
1.
Extend the expiration date one year to March 31, 2023.
2.
The total approved funding amount for the City of Phoenix will remain unchanged.
3.
Current reimbursement rates are subject to change. Rate updates can be found on the Maricopa 
County 
Department 
of 
Public 
Health 
Vaccine 
Vendor

4.
All other terms of the IGA remain unchanged.
