AMENDMENT TO CONTRACT

MARICOPA COUNTY
By and Through its
DEPARTMENT OF PUBLIC HEALTH
and
Arizona Alliance for Community Health Centers
It is mutually agreed the referenced contract is amended as follows:
I.
ATTACHMENT A, Scope of Work is revised and shall become effective on September 1,
2021.
II.
ATTACHMENT B, budget justification narrative is revised to reflect an increase in total
project costs from $801,000 to $901,000. A ($100,000 increase) for the year 3 budget
period beginning September 1, 2021 to August 31, 2022.
III.
All other terms and conditions of the original contract shall remain in full force and
effect.

AACHC - MCDPH SUD Grant
Scope of Work
This updated scope of work shall become effective September 1, 2021 through August 31, 2022
Stigma Series:
Culture Mind has several hours’ worth of footage used to create the stigma series that did not fit
into one of the pre-determined categories in the series. A 4th part to the series would be created to
compliment the previous parts and add to the conversation around stigma and substance use
disorder. This part will include segments involving family members of people in recovery and
will expand on the personal stories of people that participated in part 1 of the series. (Up to 6
videos)
Best Practice Series for NAS/SENS/Mothers with SUD:
AACHC would like to partner with Culture Mind to develop a series of short, informative videos
for providers on treatment of parents with SUD. This would include MAT during pregnancy as
well as harm reduction. We could also partner with SHIFT taskforce and Hushabye Nursery to
highlight the work they do. (Up to 15 videos)
