I. 
Project Description 
 
The law firm of Quarles & Brady LLP represents Terra Pointe I, LLC (“Terra Pointe” or 
the “Applicant”) in regard to its request for design review and to rezone the properties located at 
and near 5330 W. Union Hills Dr., Glendale, AZ 85308  (APNs: 200-25-397A, 200-25-389A and 
200-25-004F) (the “Property”) to the PAD district. See Aerial Maps attached at Tab A. 
 
The Property consists of three parcels with an aggregate gross acreage of approximately 
8.47 acres. Applicant’s existing facility, Terra Pointe Phase I, is a memory care center that 
specializes in Alzheimer’s and dementia patients and is currently operating on one parcel (APN: 
200-25-389A) while the remaining two parcels to the north and east are vacant (APNs: 200-25-
397A and 200-25-004F). See Aerial Maps attached at Tab A. With Terra Pointe Phase II, 
Applicant seeks to expand its facility onto the vacant parcels in order to provide assisted living 
services. Terra Pointe Phase II will provide these additional services in a beautiful homelike setting 
by adding an estimated 112 assisted living apartments to the existing 74 memory care beds in the 
Phase I development. The expansion will be roughly 119,000 square feet and will feature spacious 
studios, single and two-bedroom apartments and offer a multitude of amenities including 
restaurant-style dining rooms, exercise areas, recreation and other activity areas, exterior 
courtyards, covered porch areas, walking paths and more. The proposed expansion will benefit the 
surrounding community by beautifying and improving two vacant parcels, creating employment 
and volunteering opportunities and providing assisted living services to the area. 
 
II. 
Overview of Citizen Participation Plan 
 
The initial Citizen Participation Plan was approved by City Staff on December 21, 2020. 
Due to COVID-19, the Applicant team held a neighborhood meeting virtually using the Zoom 
platform on January 21, 2021 from 6PM to 6:55PM. In accordance to Planning Department 
requirements, notification letters informing the public of the upcoming virtual neighborhood 
meeting were mailed January 6, 2021. The notification letter described the proposed request and 
was mailed to all property owners located within 300 feet of the Property, the Miramonte 
Homeowners Association, and all “Interested Parties” and “Additional Parties” identified by City 
of Glendale staff. See Notification Area Map and Notification Lists attached at Tab B. The mailing 
envelopes included a return address and the City of Glendale logo label identifying the information 
as a Land Use Application. A notification affidavit packet was submitted to City staff on January 
13, 2021. See Notification Letter and Affidavit Packet attached at Tab B.  A notification sign 
approved by the City of Glendale was posted on the site on February 8, 2021 informing the public

of the upcoming Glendale Planning Commission meeting and Glendale City Council meeting date, 
time, and location. See Sign Posting Affidavit Packet attached at Tab C.  
 
III. 
Virtual Neighborhood Meeting 
 
As previously stated, due to COVID-19, the Applicant team held a neighborhood meeting 
virtually using the Zoom platform on January 21, 2021 at 6PM. Nine members of the Applicant 
team attended including those from Quarles & Brady LLP (Ben Graff, Cameron Arthur, Greg 
Loper, and Crissy Jung), Terra Pointe/Avista Senior Living (Doug Gardner, Kris Wooley, Craig 
Ahlstrom, Sanders Achen), and Reece and Rowe Architects (Stephanie Rowe). Notification letters 
were mailed to a total of 66 individuals, and a total of six individuals, aside from the Applicant 
team, attended the meeting including Glendale Senior Planner George Gehlert, Councilmember 
Lauren Tolmachoff, Art Rullo (affiliated with Avista Senior Living), Rob Kondos (affiliated with 
Avista Senior Living), Paul Sandu (affiliated with Avista Senior Living), and Yvette "Bonnie" 
Meyer (nearby property owner).  
 
The Applicant team began the meeting with a PowerPoint presentation with details about 
the proposed project. After the presentation, participants were invited to express questions or 
comments. Councilmember Tolmachoff expressed how this site used to be an "eye-sore" and a 
blighted property for years prior to the Applicant's improvements to the Property. The 
Councilmember also expressed gratitude for the partnership with the Applicant and is pleased to 
support the expansion. Property owner Yvette "Bonnie" Meyer stated she has lived nearby for a 
couple decades and agreed with the Councilmember's sentiment. Ms. Meyer asked to verify the 
existing facility's unit count and proposed additions, and wanted clarifications on the total number 
of units. The Applicant team confirmed 72 units currently exist and the total proposed units is 186 
and asked the architect to verify. Ms. Meyers also expressed concerns about traffic along 54th 
Street, and asked which street will be the primary access for vehicles. Kris Wooley from the 
Applicant team confirmed the primary access will be along Union Hills Drive and there is no plan 
to utilize or access the property on 54th Street, except for emergency access. City Planner George 
Gehlert then thanked the Applicant team for holding the presentation and expressed the proposed 
project is good for the site. 
 
No other concerns or issues were expressed to the client team from the public. If any 
additional concerns or questions arise, the Applicant team will continue to address any public 
concerns in coordination with the City of Glendale Planning Staff.

Dear Property Owner or Neighborhood Leader: 
Our firm represents Terra Pointe I, LLC ("Terra Pointe") in regard to the property located 
at 5330 W. Union Hills Dr. (Maricopa County Assessor's Parcel No. 200-25-397A, 200-25-398A 
and 200-25-004F) (the "Property") as shown on the attached aerial map. The purpose of this letter 
is to inform you we have recently filed an application to rezone the Property to the PAD district to 
expand Terra Pointe’s facility currently operating at the site.  
Terra Pointe is a local Arizona business that provides high-quality memory care services 
to the Glendale area. Established in 2017, the current Terra Pointe facility was originally 
constructed in 1981 as an assisted living community. The community remained in operation for 
20 years before going out of business and falling into disrepair. In 2015, Terra Pointe purchased 
the development and renovated it into the state-of-the-art memory care center now located at the 
site. The facility specializes in the treatment of dementia and Alzheimer’s disease in a beautiful 
and homelike setting while offering various amenities to patients such as an on-site beauty salon 
and podiatry, dental, hearing and x-ray services.    
The rezoning request will allow the expansion of Terra Pointe’s facility to two vacant, 
adjoining parcels in order to provide both assisted and independent senior living services at the 
location. Terra Pointe will invest in significant interior and exterior aesthetics to complement the 
design and style of its currently operating facility and the surrounding community and provide 
more parking spaces than required by the City of Glendale Ordinance to ensure no overflow 
parking will occur. The expansion will be a one to two-story building, and, in combination with 
the existing facility, will occupy 99,243 square feet of the total 369,143 gross square feet of the 
Property as shown in the attached site plan. The expansion will provide the neighborhood with a 
convenient, welcoming and beautiful senior living environment. Moreover, because of the 
synergies arising with the nearby high school and church, the expansion will create various 
opportunities for employment, volunteering and engagement with the area.  
Beyond informing you of the status of our application, the purpose of this letter is to invite 
you to a neighborhood meeting to discuss our proposal, which due to the COVID-19 pandemic,

will be held virtually online. Our team will show our preliminary site plan layout/design at this
