AMENDMENT TO
CONTRACT

DEPARTMENT OF PUBLIC HEALTH

Solari, Inc.
I.
The above-named contract is hereby amended as specified below:
A.
Exhibit A, Pricing, Project 2, Community Services Heat Helpline shall be increased by
$49,985, making the new not to exceed amount $180,485 for this line item. This increase
shall bring the new not-to-exceed under the Grand Total Requested to $2,978,518.
i.
This increase is a result of Increasing the number of 2025 Client Transportation
units as shown on page 36, Compensation (5.5.4), Maricopa County Heat
Helpline budget, from 3,686 to 6,510. These additional 2,824 units shall increase
the total cost by $49,985 for this line item resulting to a new Grand Total of
$180,485 for Project 2.
II.
All other terms and conditions of the original contract shall remain in full force and effect.
