I. 
The above-named contract is hereby amended as specified below: 
 
A. An additional $90,000 shall be added for the addition of a Youth Mental Health and Wellness 
program to the Scope of Work. The revised not to exceed (NTE) amount shall be updated to 
$1,674,548.   
 
C. Exhibit B - 2, Scope of work, shall be added for the January 2025 – June 2026 term as 
shown on page 2 of this amendment.   
  
II. 
All other terms and conditions of the original contract shall remain in full force and effect.

JANUARY 2025 – JUNE 2026 SCOPE OF WORK 
 
Just Communities Arizona shall implement a program to help schools and other organizations set up 
youth coalitions that focus on mental health and wellness. This includes providing curricula, training in 
schools, hosting events, and conferences focused on youth mental health and wellness.
