C-22-26-009-X-01 Amendment No. 1 Page 1 of 4 AMENDMENT NO. 1 TO AGREEMENT BETWEEN MARICOPA COUNTY ADMINISTERED BY ITS HUMAN SERVICES DEPARTMENT AND MESA UNIFIED SCHOOL DISTRICT I. Maricopa County, by and through the Maricopa County Human Services Department (“County”), and Mesa Unified School District (“District”) entered into an Intergovernmental Agreement on or about September 22, 2025. The Agreement term began July 1, 2025, through June 30, 2026. The Agreement contains an option to extend the Agreement upon written approval by the Parties. The District and the County are collectively referred to herein as the “Parties” and individually as a “Party.” The purpose of the Agreement is to establish a process in which services are provided to preschool children, including children with disabilities as defined in A.R.S. §15-761, ages three years old to kindergarten eligible five-years-olds, in compliance with Federal and State laws and regulations, and in accordance with Mesa Unified School District policy, and Improving Head Start for School Readiness Act of 2007 (42 U.S.C. §§ 9801, et seq.), by referring eligible children to the County’s Head Start program and the Head Start program shall refer eligible children to the District. II. The Parties agree to enter into this Amendment No. 1 to amend the Agreement as follows: A. Extend the Agreement term identified on page 1 of the Agreement, in accordance with section 5.0 Amendments, from July 1, 2026, through June 30, 2027, Fiscal Year 2027. B. Revise the Maricopa County Representative name and contact information and replace it with the following: County Representative Name: Mattie Lord Title: Early Education Assistant Director Maricopa County Human Services Department 234 N Central Avenue, Suite 3000 Phoenix, AZ 85004 Telephone: 602-372-4999 Email: mattie.lord@maricopa.gov C. Revise and replace Paragraph 19.0 (INSURANCE) with the following: 19.0 INSURANCE 19.1 Each Party is a Public entity and shall provide the other Party with a Certificate of Self-Insurance equal to the types and amounts specified below: General Aggregate $3,000,000 Each Occurrence Limit $1,000,000 19.2 Within ten (10) calendar days following execution of this Agreement, the District shall furnish the County, to the County Representative C-22-26-009-X-01 Amendment No. 1 Page 2 of 4 listed on page 1, with valid and complete Certificate(s) of Insurance, or formal endorsements as required in the form provided by the County, issued by the District, as evidence that policies providing the required coverage, conditions and limits as required by this Agreement are in full force and effect. Such certificates shall identify this Agreement number and title. 19.2.1 In addition to the above, the District shall provide a copy by mail to the following and Certificate holder shall be identified as: Mail COI to: Maricopa County c/o Risk Management 301 W Jefferson St., Suite 910 Phoenix, AZ 85003 19.3 Cancellation and Expiration Notice: 19.3.1 Applicable to all insurance policies required within the insurance requirements of this contract, District’s insurance shall not be permitted to expire, be suspended, be canceled, without 30 days prior written notice to Maricopa County. Such notice shall be sent directly to Maricopa County Human Services Department, Workforce Development Division and shall be mailed, or hand delivered to 234 N. Central Avenue, Phoenix Suite 3000, AZ 85004, or emailed to the Human Services representative noted in the Contract. D. Revise Section 28.0 (COLLABORATION ACTIVITIES), by removing subsection 28.8. of the Agreement in its entirety and replacing it with the following: 28.8 Community based sites operated by the County and located within the District’s boundaries: 28.8.1 Community-based locations, where Head Start Program activities are located within the District’s boundaries and are operated by either the County or independent Child Care providers, include: 28.8.1.1 Housing for Hope Bellview, 466 South Bellview Street, Mesa, AZ 85204 (County Operated) 28.8.1.2 First Presbyterian Church of Mesa, 161 North Mesa Drive, Mesa, AZ 85201 (County Operated) 28.8.1.3 First United Methodist Church of Mesa, 15 East 1st Street, Mesa, AZ 85201 (County Operated) 28.8.1.4 East Valley Institute of Technology (“EVIT”), 1601 West Main St., Mesa, AZ 85201 (County Operated) 28.8.1.5 A New Leaf / La Mesita, 2254 West Main Street, Mesa, AZ 85201 (County Operated) E. Revise Section 29.0 (FACILITY USE), by removing subsection 29.2 of the Agreement in its entirety and replacing it with the following: 29.2 During the term of this Agreement, the County shall be entitled to exclusive use of Premises at the following Facilities for service delivery of Head Start Program activities: 29.2.1 Adams Elementary School, 738 S Longmore, Mesa, AZ 85202 Room 52 Estimated SF: 938 C-22-26-009-X-01 Amendment No. 1 Page 3 of 4 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.2 Edison Elementary School, 545 N Horne Avenue, Mesa, AZ 85203 Room 5 Estimated SF: 937 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.3 Lowell Elementary School, 920 E Broadway Rd., Mesa, AZ 85204 Room 7 Estimated SF: 960 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.4 Hughes Elementary School, 630 N. Hunt Drive, Mesa, AZ 85203 Room 22 Estimated SF: 868 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.5 Lindbergh Elementary School, 930 S Lazona Drive, Mesa, AZ 85204 Rooms 313 & 314 Estimated SF: 858 each Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.6 Longfellow Elementary School, 345 S. Hall, Mesa, AZ 85204 Room 5 Estimated SF: 713 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.7 Stevenson Elementary School, 638 S 96th St., Mesa, AZ 85208 Room 30 Estimated SF: 832 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.8 Taft Elementary School, 9800 E Quarterline, Mesa, AZ 85207 Room 1 Estimated SF: 915 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. 29.2.9 Webster Elementary School, 202 N Sycamore, Mesa, AZ 85201 Rooms 35 Estimated SF: 883 Scheduled Playground Days & Hours as determined by the Parties in writing within 30 days of execution of this Agreement. III. Section II above contains all the changes to the Agreement made by this Amendment No. 1. The Agreement is amended to incorporate the changes contained in this Amendment No. 1. All other terms and conditions of the Agreement remain in full force and effect as executed by the Parties. This Amendment No. 1 is subject to and incorporates the provisions of A.R.S. §38-511. IV. The Parties have authorized the undersigned to execute this Amendment No. 1, and it shall be effective upon approval and signature by both Parties.
