3. | own 100% membership interest in Jarnstar Media, LLC, dba Dirty’s Topless Bar
& paiio, 3308 NW Grand Ave, Phx, AZ 85017, which has owned Liquor License #
06070689 since 2017.
Question 3:
| am a controlling person in Dirty’s Topless Bar & Patio (Dirty's), License #6070689 (see
above). On October 16, 2020, Dirty’s was cited by Arizona Dept. of Health Services
(ADHS) and the DLLC for non-compliance with Governor Ducey’s Executive Orders
relating to Covid-19 mitigation protocols and requirements mandated by ADHS. The
License was suspended (October 17, 2020, to October 28, 2020) pursuant to a Consent
Agreement and Order (in the Matter of Bar Spirituous liquor License #06070689, Dated
October 28, 2020), Dirty's was also 1) assessed a fine of $7,500 (the State of Arizona
subsequently paid back $7,000 to Dirty’s pursuant to House Bill 2570-24) and 2)
ordered to abide by the ADHS Attestation and Executive Orders as in place at the time
and as they were subsequently issued. Dirty’s Complied with all aspects of the Consent
Agreement and Order. ~
