AMENDMENT No. 2 To AGREEMENT Between COMMUNITY BRIDGES, INC & MARICOPA COUNTY, ARIZONA WHEREAS, Maricopa County (“County”) and Community Bridges, Inc (“Contractor”) have entered into an Agreement for the purpose of establishing a program to increase access to Medication Assisted Treatment (“MAT”), Opioid Use Disorder (“OUD”) recovery support services, and opioid prevention awareness for incarcerated individuals transitioning to the community executed November 2nd, 2022 (“Agreement”). WHEREAS, County and Contractor have agreed to renew this Agreement. NOW, THEREFORE, the parties hereby agree to the following provision: The Agreement shall be renewed for a term from November 2nd, 2024, and ending November 1st, 2025. ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED. If there is any conflict between the terms of the Agreement and this Amendment, in all such events the terms of this Amendment shall control. This Amendment is subject to cancellation pursuant to A.R.S. § 38-511.
