AMENDMENT NO. 2 
TO 
AFFILIATION AGREEMENT 
BETWEEN 
MARICOPA COUNTY 
ADMINISTERED BY ITS 
HUMAN SERVICES DEPARTMENT 
AND EAST VALLEY INSTITUTE OF TECHNOLOGY 
 
I. 
Maricopa County (“County”) administered by its Human Services Department 
(“Department”), and East Valley Institute of Technology District #401 (“EVIT”) fully 
executed a non-financial Affiliation Agreement (Agreement) on or about November 21, 
2023. The term of the Agreement is for a three-year period from date of execution 
(November 15, 2023, through November 15, 2026.) The purpose of the Agreement is to 
provide internships and other hands-on training experiences for EVIT’s Program students 
(“Students”) as part of their overall education. EVIT and the County are collectively 
referred to as the “Parties” and individually as a “Party.” 
 
The Parties fully executed Amendment No. 1 on or about June 21, 2024. The Amendment 
expanded EVIT’s educational internship hand-on-training experiences for students 
enrolled in Nursing, Dental, Medical Assistants, Practical Nursing, Emergency Medical 
Technician (EMT), Mental and Social Health Technician. The Amendment added the 
Provisions Required By Law clause. 
 
II. 
The Parties now agree to enter into this Amendment No. 2 to amend the Agreement as 
follows: 
 
A. 
To modify the Agreement from a non-financial to financial Agreement. 
 
B. 
The County shall provide EVIT with a not-to-exceed amount of $8,000 for the 
purchase of Early Education training curriculum and supplemental resources and 
aides for students enrolled in the Early Childhood Education Program. The funding 
for this Amendment is provided by Human Services Department appropriated 
County General Funds – Community Solutions Funds. 
 
C. 
Add the following Sections to the Agreement: 
 
27. 
FUNDING 
The period of funding availability is July 24, 2024, through December 31, 
2024. 
 
28. 
METHOD OF INVOICING 
28.1 
The Agreement amount shall be on a cost reimbursement basis and 
shall not exceed $8,000. 
28.2 
The County shall reimburse on a net “0” payment standard after 
invoices and backup documentation has been reviewed and 
approved. 
28.3 
Invoices shall be submitted to HSDFinance@Maricopa.gov
