About the Grant Program Application:
The Salt River Pima-Maricopa Indian Community 12% Gaming Grant Program (“Program”) application
will serve to provide a comprehensive review of the programming requests for Arizona cities, towns,
counties, non-municipal public service organizations, and non-profit organizations selected to apply for
grant funding. It is not the intent of the Program to award grants that would benefit individuals, be used
as sponsorships, be used for religious activities, primarily fund the indirect or administrative costs of a
program/organization, or be used for the direct benefit of the Salt River Pima Maricopa Indian
Community.

The Arizona Diamondbacks Foundation was formed in 1997. The mission of the Arizona
Diamondbacks Foundation is to support three main areas of need, including homelessness, indigent
healthcare and children's programs of all types including education, youth baseball and softball field
renovations, outreach and equipment. In addition, the Foundation has created strategic programs
that address the needs of our community including veterans, police, firefighters and teachers.

The Arizona Diamondbacks Foundation Board of Directors includes 7 appointees from the Arizona
Diamondbacks and 6 appointees from Maricopa County. The Foundation's leadership includes
Executive Director, Debbie Castaldo and Director of Operations, Tara Trzinski.

Provide a brief description of the project being submitted to the 12% SRPMIC Grant program. In
the description provide a summary of the relevant experience, administrative activity, and
program development related to the grant request.
The D-backs and United Health Care are teaming up with Kitchen on the Street to create the Fresh on
Deck Mobile Market to bring exciting bright bites food option to kids! Our mission will include
bringing healthy options to at-risk kids in communities, schools and local neighborhoods. Our
passion is meeting kids and families where they live, regardless of their barriers to healthier options
for their kids!
Capturing the cool experience of visiting a food truck, the Fresh On Deck Mobile Market will
encourage kids to crunch it, bite it, munch it, try it!! We estimate that the brightly painted truck will
deliver thousands of servings of fruits and vegetables to school-aged children in communities across
Arizona. Children will have the opportunity to sample and taste all sorts of fruits and vegetable
options including fresh produce, salads, fruit smoothies and more.
Additionally, by teaming up with St. Mary’s Food Bank, Fresh on Deck Mobile Market with distribute
food boxes including fruits and vegetable options to families in these same communities.
Whether at schools or community centers, Fresh on Deck Mobile Market will support health habits by
providing healthy options and fun recipes. Additionally, we will provide much needed food assistance
in local communities. A healthy diet produces better academic outcomes for students! Now is a
critical time to support our most vulnerable families with Fresh on Deck!
What percentage of the grant request are Direct Costs of the programming and Indirect Costs,
respectively? All of the funds will be used to purchase fruits and vegetables and assist with the
costs of operating the truck and delivering donated food to local schools, neighborhoods and
community centers.

Funding for project/programs may be based on available funding. As such, if funding were not

By execution of the 12% Gaming Contribution Grant Application the undersigned agrees that the
information contained in the Application is true to the best of the Applicants knowledge. The
Applicant shall notify the SRPMIC if any information in this Application changes.
The Application shall be signed by the senior elected official of the local government, or in the case of
a non-profit entity, the Chief Executive Officer authorizes to commit the non-profit entity to apply for,
accept, and implement an award from the SRPMIC utilizing 12% Gaming Contributions.
