Dear Mr. Bohnee,

Native American Connections (NAC) hereby submits its 12% Gaming Contributions grant
application (CY2021) to the Salt River Pima-Maricopa Indian Community (SRPMIC).

NAC is requesting $50,000 in grant funding from SRPMIC to support NAC’s Affordable and
Permanent Supportive Housing (PSH) communities, particularly those also focused on providing
access to specialized Native American cultural/traditional healing practices. This includes
Dunlap Pointe, a new PSH community co-located in a North Phoenix (Sunnyslope) campus
adjacent to NAC’s new Patina Mountain Preserve Wellness Center, both of which will open in
Fall 2020.

We are grateful to the SRPMIC’s 12% Gaming Grant Committee for selecting and inviting NAC
to submit an application for the 2021 grant cycle.
