Opportunity4Kids (Tax ID 46-3403537) respectfully submits a grant application to 
SRPMIC for $30,000 to supplement and support our ongoing endeavors. We can 
only imagine the impact COVID-19 had on your operations and are truly grateful 
to even be considered for funding again this year.  
We were graciously granted funds in 2016, 2018, 2019 and most recently 2020; 
have put awarded dollars to good use, extending opportunities to 120 youth who 
would not otherwise have them. As the support from SRPMIC continues, our 
reach grows within Maricopa County and creates a bigger impact. We thank you 
for this!  
Our goal is to continue long term, measurable impact in Maricopa County by 
providing at least 100 underprivileged youth with opportunities to participate in 
out-of-school time activities such as afterschool, online, or summer camp 
programs and extracurriculars.

The Salt River Pima-Maricopa Indian Community 12% Gaming Grant Program (“Program”) application 
will serve to provide a comprehensive review of the programming requests for Arizona cities, towns, 
counties, non-municipal public service organizations, and non-profit organizations selected to apply for 
grant funding.  It is not the intent of the Program to award grants that would benefit individuals, be used 
as sponsorships, be used for religious activities, primarily fund the indirect or administrative costs of a 
program/organization, or be used for the direct benefit of the Salt River Pima Maricopa Indian 
Community.

1. We would prioritize by serving those most in need first, focusing on those that only fall at

2. We can also adjust the amount per Scholarship to proportionally fit the amount awarded, thus still
serving an increased number of children and making a dramatic impact.

ATTACHMENT A 
Founded in 2013, Opportunity4Kids began as Equipment4Kids- providing equipment and 
uniforms to kids on the sidelines without resources to play.  
In 2015, the Opportunity4Kids Board acknowledged that youth need more than cleats to 
participate, and not every kid wants to play a sport. Opportunity4Kids then expanded its 
focus to include all out-of-school time activities such as afterschool programming, 
summer camp and extracurriculars.  
Excitedly, Opportunity4Kids celebrated our 7-year anniversary this fall. What started as 
14 Scholarships quickly turned into over 100 annually and community programs 
impacting thousands!  
Opportunity4Kids focuses on serving youth and their families living throughout Maricopa 
County. Per the US Census Bureau’s American Community Survey, in Maricopa County 
there were 1,014,531 children under the age of 18 years old in households. The 
percentage of children living in households at or below the poverty line is 24.4% and it 
was determined that 28.7% of children are living in households with supplemental security 
income, cash public assistance income or receiving food stamp benefits.  
Oppotunity4Kids strives to support the 1-in-4 children in Maricopa county that need 
resources to participate by providing necessary Scholarships, equipment and uniforms! 
Already in September of 2020, Opportunity4Kids has been able to put ¾ of our CY2020 
SRPMIC funding in action- providing 98 Individual Scholarships to Maricopa County 
Youth, and we plan to keep going! COVID-19 dramatically impacted operations, halting 
all Scholarships being awarded and processed and changing the activity environment as 
we know it. Opportunity4Kids quickly pivoted to stay active and provide online activities 
and programming, paid for by other funding sources. As the state restrictions are shifting, 
we are jumping back in to in-person activities and therefore Scholarships, as well as 
addressing technology needs. We anticipate even more need than prior years in CY2021 
as parents are faced with a new environment for childcare, schooling and activities.

mental and social development; and allow equal opportunities for children in need.

ATTACHMENT C 
 
With our project, Leveling the Playing Field in Maricopa County!, Opportunity4Kids’ goal is to 
increase participation in out-of-school time opportunities for children who would otherwise not 
be able to afford. Opportunity4Kids will facilitate involvement in out-of-school time activities, 
such as afterschool and online programs, summer camps and extracurriculars. This will be done 
by providing Individual Scholarships to pay for registration fees required to enroll and/or 
equipment and uniforms.  
 
It is through word-of-mouth within communities of need, or by case workers and coaches, that 
Opportunity4Kids will identify underprivileged youth who would benefit from out-of-school time 
Scholarships. The Parent/Guardian/Case Worker of qualified children will be required to 
complete an online application to be vetted by Opportunity4Kids’ Staff and then submitted to 
Opportunity4Kids’ Scholarship Approval Committee for decision. The online application 
identifies the true need of applicants by requesting individual health and situational information, 
as well as household financial data, to determine that Scholarships are awarded to those most 
disadvantaged. Youth Scholarships are evaluated on a case-by-case basis throughout the year, 
centered around school seasons: Spring, Summer, Fall Winter. 
 
Through the Scholarship process, Opportunity4Kids’ Operations Director will utilize a tracking 
system to record information on children and their families. The expectation is that 98% of Youth 
provided Scholarship will live in households that fall at or within 1.5% of the federal poverty line 
and at least 75% also receive additional government assistance. To determine the success of 
Scholarships provided, Opportunity4Kids asks the Parent/Guardian/Case Worker to complete a 
post survey to analyze individual progress and development. At the conclusion of Scholarship 
activity, we anticipate 90% will show increase in self-esteem and social skills due to participation 
in programming. 
 
Opportunity4Kids has established a supportive network of partners throughout Maricopa County 
with nonprofits and recreational organizations that provide positive out-of-school time activities 
such as: Skate After School, Eagles Taekwondo, Phoenix Center for the Arts, Arizona Animal 
Welfare League, Ballet Arizona, Summer Camp of Arizona, ICAN, Lights Camera Discover, 
NSFC Soccer League, Boys and Girls Clubs of the Valley and many more.   
 
These organizations support Opportunity4Kids’ goal of increasing participation by discounting or 
reducing registration fees required to participate, thus facilitating the Scholarship of more 
children for a longer duration. 
 
If provided funding from SRPMIC, it would go directly to program Scholarships, 
increasing Opportunity4Kids impact in Maricopa County for at least 100 children!
