AMENDMENT TO CONTRACT
Between MARICOPA COUNTY
By and Through DEPARTMENT OF PUBLIC HEALTH
And DANIELS MOVING AND STORAGE
I.
Due to the continuing COVID-19 response, the contract term is extended for an
additional six (6) months from March 9, 2021 through September 8, 2021. This is
the 2nd six-month extension beyond the original maximum 5-year term, making
the total term 6 years from September 9, 2015 through September 8, 2021.
II.
Increase the total not to exceed amount over the life of the contract to $440,000.
This includes the initial budget of $70,000 that was included in the initial 3-year
term.
III.
All other terms and conditions of the original contract shall remain in full force
and effect.
