To
MEMORANDUM OF UNDERSTANDING
Between 
MAGELLAN COMPLETE CARE OF ARIZONA
&
MARICOPA COUNTY, ARIZONA
WHEREAS, Maricopa County (“County”) and Magellan Complete Care of Arizona (“Contractor”) have entered into an 
Agreement for the purpose of establishing collaborative protocol executed July 31st, 2019 (“Agreement”).
WHEREAS, County and Contractor have agreed to modify the Agreement by changing certain terms and conditions; 
NOW, THEREFORE, the parties hereby agree to the following provisions:
1.
The Agreement shall be renewed for a term from July 31st, 2020 and ending July 30th, 2021. 
2.
The Director of Maricopa County Correctional Health Service is authorized to execute future amendments 
to the Agreement unless, in the opinion of the Director, a proposed change is so substantial that approval of 
the Maricopa County Board of Supervisors is required.
ALL OTHER TERMS AND CONDITION REMAIN UNCHANGED. If there is any conflict between the terms of 
the Agreement and this Amendment, in all such events the terms of this Amendment shall control.
This Amendment is subject to cancellation pursuant to A.R.S. § 38-511.
IN WITNESS WHEREOF, this Contract Amendment is executed on the date set forth below when executed by both 
Parties Authorized Representative.
