The stipend is for Site Coordinator time and assistance in the organization of the STAND coalition. The stipend will be disbursed within the period of August 1, 2020 and June 30, 2021 to Peoria Unified School District for Site Coordinator. The stipend will be disbursed in 2 separate installments of $1,000, once at the beginning of the school year (August 2020) and once at the end of school year (May 2021) upon successful completion of the program requirements. An invoice is required from the PUSD business office to be transmitted to Maricopa County Public Health Department when stipend payments are due. Minimum Site Coordinator Requirements: Recruit youth to participate in the STAND youth coalition. Arrange for online meetings including sending invites and other organization until it is safe and approved to meet in person. When safe and approved to meet in person, arrange for a meeting location at the school and provide notice to youth and MCDPH coordinators if meeting times or locations are changed/canceled. Meet with MCDPH STAND Coordinator for an all coordinators meeting, at least two times per year. Arrange transportation for the youth for off-campus events if possible. Attend and actively participate in STAND Youth Coalition meetings. Facilitate communications between students and the MCDPH STAND Coordinator. Arrange for coalition members to conduct peer-to-peer education presentations. Participate in an orientation to review roles and expectations. Actively participate with youth coalition members to conduct activities and events. Provide suggestions and confirm STAND action plan for school year with MCDPH Coordinators. Provide feedback to the MCDPH staff related to successes and challenges: help draft next steps to address any concerns or issues. Assist the MCDPH STAND coordinator in the process of making STAND an official club at the high school. Help recruit youth for the Counter Strike and FDA inspection program. Participate in End of the Year STAND evaluation. Attend one youth policy related event such as meeting with elected officials. Attend one Arizona Department of Health Services sponsored youth training/conference. CONTRACT NO.: C-86-17-041-3-05 AMENDMENT NO. 5 3 Inform MCDPH at least a week before if unable to chaperon youth. Items for STAND Coalition Activities MCDPH will purchase the items for the coalition through the MCDPH approved vendors and process. This process could take a few weeks and it is recommended that the Site Coordinators plan items needed in advance. Items/events to support and facilitate STAND youths’ effort o Coalition specific shirts (with specific color and Logo) for STAND members o Water and snacks o Project specific supplies (example – brushes, paints, etc.) o Mid-year and End of year celebrations o Promotional items o Summer conference theme supplies o Educational items
