AMENDMENT NO. 1 TO THE  
CONTRACT 
BETWEEN  
MARICOPA COUNTY  
ADMINISTERED BY ITS  
HUMAN SERVICES DEPARTMENT  
AND 
CIRCLE THE CITY 
 
I. 
Maricopa County (“County”), administered by its Human Services Department, and Circle 
the City, a non-profit, community-based organization (“Contractor”) entered into a financial 
agreement (“Agreement”), which was fully executed on or about June 24, 2020, for nursing 
services for individuals experiencing homelessness who are COVID-19 positive or 
symptomatic at the County temporary housing facility (“Facility”). The Agreement Start 
Date is May 11, 2020, and the Termination Date is September 30, 2020. In accordance 
with the terms of the Agreement, the County will pay the Contractor an amount not to 
exceed $955,584 in FY20 Maricopa County General Funds for these services. The County 
and Contractor collectively are referred to as the “Parties.”  
 
II. 
The Parties agree to enter into this Amendment No. 1 to amend the Agreement as follows: 
 
A. 
Extend the Agreement Termination Date from September 30, 2020, to December 
30, 2020. 
 
B. 
Revise Section 2.0 (Purpose), Subsections 2.1 and 2.2, by replacing them with the 
following: 
2.1 
To provide medical care and nursing care services (“Services”) to 
individuals experiencing homelessness who are either COVID-19 positive 
or symptomatic (or both) at the Phoenix Inn facility (“Facility”).  
2.2 
The Facility is located at 8152 North Black Canyon Highway, Phoenix, 
Arizona 85051. 
 
C. 
Revise Section 3.0 (Contractor Responsibilities), Subsection 3.1, by replacing it 
with the following: 
3.1 
The Contractor shall provide the following at the Facility: 
3.1.1 Services twenty-four (24) hours per day and seven (7) days per 
week. 
3.1.2 Staff to oversee the medical and nursing care of the patients at the 
Facility.  
3.1.3 On-call Services based on Exhibit A.  Staff numbers shall be based 
on a daily census of individuals taken by the Contractor according 
to Exhibit A. 
 
D. 
Revise Section 10.0 (Funding, Invoicing, and Payment) by adding the following 
subsections: 
10.4 
The final invoice must be received by January 15, 2021. 
10.5 
Funding for Amendment No. 1 is provided by Catalog of Federal Domestic 
Assistance (CFDA) 21.019 Coronavirus Aid, Relief, and Economic Security 
Act (CARES Act), Public Law 116-136 
 
E. 
Revise Exhibit A to include the additional service tiers.

III. 
The foregoing paragraphs contain all the changes made by this Amendment No. 1. All 
other terms and conditions of the Agreement shall remain the same and in full force and 
effect as approved. 
 
IV. 
The Parties have authorized the undersigned to execute this Amendment No. 1 on their 
behalf. 
 
V. 
This Amendment No. 1 shall be effective upon approval and signature by both Parties.
