Contract No. C-22-22-109-X-00 AFF-002-03 Amendment No. 3 Page 1 of 2 AMENDMENT NO. 3 TO AFFILIATION AGREEMENT BETWEEN ARIZONA BOARD OF REGENTS ON BEHALF OF THE UNIVERSITY OF ARIZONA AND MARICOPA COUNTY I. Maricopa County (“County”) administered by its Human Services Department and Arizona Board of Regents on behalf of the University of Arizona (“School”) entered into a financial Affiliation Agreement (“Agreement”) on or about December 20, 2022. The purpose of the Agreement is to identify Student Nurses (“Students”) to participate in the County’s Student Nurse Practice Readiness Program (the “Program”). The Program will provide Students with educational opportunities to enter into hands-on training experiences as part of their overall education and which may qualify for academic credit as determined by the School. The Agreement term began August 22, 2022, through December 31, 2022. The County provided the School with $6,000 for the Nurse Readiness Program. The School and the County are collectively referred to as the “Parties” and individually as a “Party.” The Parties executed Amendment No. 1 on or about October 18, 2023. The Amendment No. 1 extended the Agreement from December 31, 2022, through December 31, 2023. The Parties acknowledge that 68 Students were placed in the Program during the May 1, 2023, through the December 31, 2023, timeframe. The County provided the School with $27,200 to provide academic oversight of Student’s capstone experience. The Amendment also addressed Agreement required language. The Parties executed Amendment No. 2 on or about December 21, 2023. The purpose of the Amendment No. 2 was to extend the Agreement term through June 30, 2024, for the School to place up to 40 Students in the Program for the extension term. The County provided the School with $400 per BSN Student for faculty oversight at the end of each Student’s successful completion of Program activities. The County provided the School with a not-to-exceed amount of $16,000 for academic oversight of Student’s capstone experience. II. The Parties now agree to amend the Agreement by the Amendment No. 3 as of the last date of signature of the Parties below (“Amendment No. 3 Effective Date”) as follows: A. Revise paragraph 14. (Notices) to add point of contact for Maricopa County and to change the School’s notice address. Extend the Agreement term from June 30, 2024, through December 31, 2024. C. The School will place up to 45 Students in the Program through the December 31, 2024, timeframe. D. The County shall provide the School with funds for the academic oversight of Student’s capstone experience as listed below: 1. $400 per BSN Student’s for faculty oversight at the end of each Student’s successful completion of Program activities. ($400 x up to 45 Students) 2. Total funds shall not exceed $18,000 for the extended Agreement term. 3. Unexpended funds from previous terms shall not carry over into this extension period. III. Section II above contains all the changes made by this Amendment No. 3. All other terms and conditions of the Agreement and previously approved Amendments shall remain the same and in full force and effect as approved and amended. IV. The Parties have authorized the undersigned to execute this Amendment No. 3 on their behalf and shall be effective on the Amendment No. 3 Effective Date.
