Public Health Emergency Preparedness 
It is mutually agreed that the Intergovernmental Agreement referenced is amended as follows:

Pursuant to the Terms and Conditions, Provision Six (6) Contract Changes, Section 6.1 Amendments, Purchase Orders and
Change Orders, the following changes are made under this Amendment Nine (9):
1.1 
The Scope of Work, Provision Three (3) Tasks, Section 3.3. Annual Performance Requirements is revised to include the
Scope of Work of this Amendment Nine (9); and  
1.2 
The Price Sheet is revised to include the Price Sheet of this Amendment Nine (9). 
**All other provisions of this agreement remain unchanged.**

Pursuant to A.R.S. § 11-952, the undersigned public agency attorney has determined that 
this Intergovernmental Agreement is in proper form and is within the powers and authority 
granted under the laws of Arizona 
This Intergovernmental Agreement Amendment shall be effective 
the date indicated.  The Public Agency is hereby cautioned not to 
commence any billable work or provide any material, service or 
construction under this IGA until the IGA has been executed by 
an authorized ADHS signatory.

3.3.3.  City Readiness Initiative (CRI) Jurisdictional Requirements (Maricopa and Pinal Counties): 
 
3.3.3.1. 
Participate in one medical countermeasure (MCM) Dispending full-scale exercise or mass 
vaccination full-scale exercise. The exercise will include at least one Point of Dispensing (POD) in 
each CRI planning jurisdiction; 
 
3.3.3.2. 
Attend quarterly Action Plan meetings each budget period; 
 
3.3.3.3. 
Participate in scheduled Operational Readiness Review (ORR); 
 
3.3.3.4. 
Complete three (3) annual drills by June of the budget period; 
 
3.3.3.5. 
Facility Setup; 
 
3.3.3.6. 
Site activation; and  
 
3.3.3.7. 
Throughput estimation is now a component of the dispensing full-scale exercise (FSE). If a site 
does not participate in the dispensing FSE, oral MCM throughput will be measured and information 
submitted at least once during the five (5) year project period.
