NON-PROCUREMENT CONTRACT 
FOR FEDERALLY QUALIFIED HEALTH CENTERS 
By and between 
MARICOPA COUNTY  
By and Through 
DEPARTMENT OF PUBLIC HEALTH 
And 
ARIZONA ALLIANCE FOR COMMUNITY HEALTH CENTERS 
 
I. 
The above-named contract is hereby amended as specified below: 
 
A. Section 3.6 shall be changed to read: “As of May 2024, all funds have been obligated. All funds 
must be disbursed/expended by September 30, 2026.”  
 
B. Section 10.8 shall be changed to read: “Earned interest on funds under Section 3.6 will be returned 
to Maricopa County. Projected earnings through October 31, 2024, should be received by 
December 31, 2024. Any future earned interest should be returned annually by September 30 until 
the end of the contract period.”    
         
II. 
All other terms and conditions of the original contract shall remain in full force and effect.
