AMENDMENT TO INTERGOVERNMENTAL AGREEMENT (IGA)

MARICOPA COUNTY by and through its  
DEPARTMENT OF PUBLIC HEALTH, DENTAL SEALANT SERVICES 
and DYSART UNIFIED SCHOOL DISTRICT  
 
I. 
The above named IGA is hereby amended as specified below. 
 
A. Extend the term for one year (1) to begin July 1, 2020 and ending June 31, 
2021 unless terminated by either party.   
 
II. 
All other terms and conditions of the original IGA shall remain in full force and 
effect.
