TOTALS (sum of | and J) Ss 89,632 AGREEMENT NO: C-86-20-024-3-01 AMENDMENT NO: 1 ATTACHMENT A -BUDGET NARRATIVE For Arizona State Board of Pharmacy For the Period September 1, 2020 to August 31, 2021 Budget Justification The total budget requested to support implementation of the Arizona Board of Pharmacy’s Prescription Monitoring Program is $89,632. All non-applicable cells in the budget summary remain blank. Additional details of the budget can be found below. Salaries and Wages: The Arizona State Board of Pharmacy (ASBP) requests salaries for one 1-FTE Epidemiologist at $55,000. The epidemiology position will collect, tabulate, and interpret required data and provide the necessary detailed reports. The total line item (A) requested of federal grant funds for personnel salaries is $55,000. Fringe Benefits: The ASBP applies a fringe benefits rate of 42.45% that covers the following items: Medical (21.0%); Retirement (12.04%); FICA (6.2%); Medicare (1.45%); Employee-related expenses, including Basic life, LTD, SUTA, and Worker’s Compensation (1.79%). This includes $23,348 for the Epidemiologist for one year. The total line item (B) requested of federal grant funds for fringe benefits is $23,348. Consultant Costs: The ASBP is not currently seeking consultant costs. Equipment: The ASBP is not currently seeking equipment costs. Supplies: Software licensing costs are estimated at $950. This includes Delimit software to open large sized PMP files provided by the vendor ($50), as well as an annual license for Tableau software to perform statistical analysis and create visualizations to document program activities, measure progress, and create statistical and data visualization reports and presentations ($900). Travel: Annual travel is estimated at $1,000. This covers the cost of sending two team members to present localized PDMP data to Maricopa County partners and agencies and to guide how the data may be interpreted by local community organizations. Other Categories: No funds are requested for other categories. Contractual: The software vendor will continue to provide a data package to provide county-level aggregate data for “Dangerous Combinations.” These data provide needed information to Maricopa County stakeholders related to overdose trends and prescription drugs. The annual maintenance cost for the data package is $1,500. Indirect: Indirect charges are based on the federal de minimis rate of 10% on the Total Direct Labor (personnel + fringe benefits, totaling $78,348, generating $7,835 in indirect charges.
