Agreement No C-29-10-006-3-00 
Between 
The Arizona Board of Regents for and on behalf of Arizona State University 
And 
Maricopa County through the Office of the Medical Examiner (MCOME) 
 
The Agreement originally dated July 29, 2010 and amended on October 10, 2018 and April 22, 2020, 
is further amended as follows: 
 
The purpose of this amendment is to include the collection of data pertaining to the Center for 
Disease Control and Prevention’s rapid detection component under the State Unintentional 
Drug Overdose Reporting System (SUDORS) in the state of Arizona (AZ-SUDORS). This 
component is to more rapidly detect opioid overdose outbreaks or sharp increases in opioid 
overdose deaths.  In addition to the Scope of Work stipulated in the original Agreement under 
15. SCOPE OF WORK, data to be collected includes specific clinical or scene evidence, 
specific information gathered in interviews conducted by MCOME Investigators, and/or 
toxicology results that indicate suspected opioid overdose deaths. 
 
15. SCOPE OF WORK is further modified whereby ASU agrees to select and refer to MCOME 
a graduate-level student or ASU employee to gather all the information agreed to in the original 
Agreement and the three Amendments.  MCOME retains the right to reject any individual 
referred by ASU.  ASU also agrees to provide a laptop and required accessories 
(carrying/protective case; wireless keyboard and mouse; docking station) for the purposes of 
collecting this research.  The individual accepted by MCOME will be allowed limited access to 
MCOME systems and must comply with the county requirements for use of county property.  If 
necessary, the individual may be required to sign a non-disclosure agreement regarding 
information obtained that is confidential or proprietary to the MCOME.  The MCOME retains 
the right to remove the individual from MCOME premises for any reason at any time. 
 
All other provisions set forth in the original Agreement and first and second Amended Agreements shall 
remain the same. 
 
IN WITNESS WHEREOF, the parties have caused this amendment to be executed by their duly 
authorized representatives. The amendment is effective as of the date of last signature by the authorized 
representatives.
