AMENDMENT TO 
WORK EXPERIENCE AGREEMENT 
BETWEEN 
MARICOPA COUNTY 
ADMINISTERED BY ITS  
HUMAN SERVICES DEPARTMENT 
AND

I. 
Maricopa County (“County”) administered by its Human Services Department and 
__________________ (“Partner”) entered into a non-financial Work Experience (WEX) 
Agreement (“Agreement”) on or about __________. The purpose of the Agreement is to enroll 
eligible Workforce Innovation and Opportunity Act (WIOA) program participants in Work 
Experience Training. The Agreement includes options to extend the Agreement for two (2) one-
year terms. The County and the Partner are referred to  collectively as the “Parties.” 
 
The Parties fully executed Amendment No. 1 (Extension Option #1) on or about ____________, 
2019, which extended the term of the Agreement through _____________, 2020. 
 
II. 
The Parties agree to enter into this Amendment No. 2 (Extension Option #2) to the Agreement to 
amend the Agreement as follows: 
 
A. 
Extend the End Date of the Agreement from ____________, 2020, to ______________, 
2021. 
 
B. 
Add the following new section: 
 
17.0 
WRITTEN CERTIFICATION PURSUANT TO A.R.S. § 35-393.01 
If the Partner engages in for-profit activity and has 10 or more employees, and if 
this Agreement has a value of $100,000 or more, then the Partner certifies it is not 
currently engaged in, and agrees for the duration of this Agreement not to engage 
in, a boycott of goods and services from Israel.  This certification does not apply 
to a boycott prohibited by 50 U.S.C. § 4842 or a regulation issued pursuant to 50 
U.S.C. § 4842. 
 
III. 
This Amendment is subject to the provisions of A.R.S. §38-511.  
 
IV. 
The foregoing paragraphs contain all the changes made by this Amendment No. 2. All other terms 
and conditions of the Agreement remain the same and in full force and effect as approved and 
amended. 
 
V. 
The Partner has authorized the undersigned to execute this Amendment No. 2 on its behalf. 
 
VI. 
This Amendment No. 2 shall be effective upon approval and signature by both Parties.
